桡动脉压迫器不同减压时间对冠状动脉介入术患者的影响

Effect of different decompression time of radial artery compressor on patients undergoing coronary intervention

目的探究桡动脉压迫器不同减压时间对冠状动脉介入术患者的影响。方法选取2017年6月～2018年5月在我院进行冠状动脉介入术并应用TR-Band止血气囊的160例患者作为研究对象,按照术后减压时间不同分为2 h组(n=50)、4 h组(n=55)和6 h组(n=55)。观察比较三组的桡动脉搏动、局部创口有无不适症状和并发症。结果三组术后穿刺部位渗血血肿发生率比较,差异无统计学意义(P>0.05);2 h组的肢端肿胀麻木发绀、创口疼痛、迷走神经反射、桡动脉闭塞发生率明显低于4 h组和6 h组(P<0.05);2 h组的术后数字疼痛评分(NRS评分)明显低于4 h组和6 h组(P<0.05)。结论行冠状动脉介入术后患者术后使用止血气囊压迫穿刺部位的止血时间为2 h的可行性较高,安全性较高,具有较少的术后并发症,值得采纳。

Objective To explore the effect of different decompression time of radial artery compressor on patients undergoing coronary intervention. Methods A total of 160 patients with TR-Band hemostatic balloon who underwent coronary artery interventional therapy in our hospital from June 2017 to May 2018 were selected as the study subjects. They were divided into 2-hour group (n=50), 4-hour group (n=55) and 6-hour group (n=55) according to the time of decompression after operation. The radial artery pulsation, local wound discomfort and complications were observed and compared among the three groups. Results There was no significant difference in the incidence of bleeding and hematoma at puncture site between the three groups (P>0.05). The incidence of swelling, numbness, cyanosis, wound pain, vagal reflex and radial artery occlusion in the 2-hour group were significantly lower than those in the 4-hour group and 6-hour group (P<0.05). The NRS score of 2-hour group was significantly lower than that of 4-hour group and 6-hour group (P<0.05). Conclusion It is feasible and safe to compress the puncture site with a hemostatic balloon for 2 hours after coronary artery interventional surgery. It has fewer complications and is worth adopting.
