锁定钢板配合带线锚钉治疗肱骨近端骨折疗效分析

Effects of Locked Plate Combined with Suture Anchor on Proximal Humeral Fractures

目的探讨锁定钢板配合带线缝合锚钉治疗肱骨近端骨折的有效性和安全性。方法选取2015年7月—2017年7月创伤骨科住院治疗、诊断为肱骨近端骨折患者63例,均为新鲜闭合性骨折,所有患者均行切开复位锁定钢板内固术,术中锚钉辅助复位大、小结节和修补肩袖,术后定期复查X线并于末次随访使用Neer肩关节评分法评价患者功能情况,期间记录患者不良事件。结果患者得到9～26个月(平均15. 3个月)随访,Neer评分:优24例,良30例,可6例,差3例。其中3例出现肱骨头坏死,1例患者螺钉穿出肱骨头,2例患者内翻畸形。结论锁定钢板配合缝合锚钉治疗肱骨近端骨折固定强度可靠,可允许患者更早进行功能锻炼,临床疗效确切。

Ojbective To investigate the efficacy and safety of locked plate combined with suture anchor in the treatment of proximal humeral fractures.Methods 63 patients with close and fresh proximal humeral fractures,who were admitted in our hospital from July 2015 to July 2017,were selected for the study.All patients underwent open reduction and locked plate internal fixation.Anchor-assisted reduction of tuberosity and repair of rotator cuff were conducted during the operation.X-ray examination was performed regularly after the operation.Neer Shoulder Scoring was used to evaluate the function of the patients at the last follow-up,and adverse events were recorded during the period of follow-up.Results The patients were followed up for 9 to 26 months,(averaged 15.3 months.Neer Shoulder Scoring showed 24 cases were excellent,30 cases were good,6 cases were fair and 3 cases were poor.Among them,3 cases of patients developed with necrosis of the humeral head,the screwed cut out in 1 patient,and 2 cases of patients had varus.Conclusion Locked plate combined with suture anchor is reliable and effective in the treatment of proximal humeral fractures.
