电凝止血与镜下缝合止血在腹腔镜下卵巢囊肿剥除术患者中的效果比较

Electrocoagulation hemostasis versus laparoscopic suture hemostasis in treatment of patients with laparoscopic ovarian cystectomy

目的探讨电凝止血与镜下缝合止血对腹腔镜下卵巢囊肿剥除术患者性激素水平及卵巢功能的影响。方法将188例卵巢囊肿剥除术患者随机分为对照组与观察组,每组94例。观察组术中予以缝合止血,对照组采取电凝止血法。比较2组患者康复状况、性激素水平。结果 2组手术时间、术后下床时间比较无显著差异(P> 0. 05)。观察组术中失血量显著多于对照组(P <0. 05)。2组术后卵泡刺激素及促黄体生成激素水平均显著高于术前水平,且观察组显著低于对照组(P <0. 05),而观察组雌二醇水平显著高于对照组(P <0. 05)。2组术后排卵异常、卵巢早衰比例无显著差异(P> 0. 05),而观察组经量过少及经期延长比例显著低于对照组(P <0. 05)。结论与电凝止血法相比,镜下缝合止血对腹腔镜下卵巢囊肿剥除术患者术后性激素水平以及卵巢功能的影响较小,但出血量略多。

Objective To explore the effects of electrocoagulation hemostasis and laparoscopic suture hemostasis on the levels of sex hormones and ovarian function in patients with laparoscopic ovarian cystectomy. Methods A total of 188 patients with ovarian cystectomy were randomly divided into control group and observation group, with 94 cases in each group. The observation group was treated with intra-operative suture hemostasis, while the control group was treated with electrocoagulation hemostasis. Rehabilitation condition and levels of sex hormones were compared between two groups. Results There were no significant differences in the operation time and postoperative time to bed-off between the two groups ( P >0.05). The intra-operative blood loss of observation group was significantly more than that of the control group ( P <0.05). After operation, the levels of follicle stimulating hormone and luteinizing hormone in both groups were significantly higher than those before operation, and these indexes in the observation group were significantly lower than the control group ( P <0.05). The estradiol level in the observation group was significantly higher than that in the control group ( P <0.05). There were no significant differences in the ratios of abnormal ovulation and premature ovarian failure between the two groups ( P >0.05), but the ratios of hypomenorrhea and prolonged menstrual period in the observation group were significantly less than those in the control group ( P <0.05). ConclusionCompared with electrocoagulation hemostasis, laparoscopic suture hemostasis cause less effects on postoperative level of sex hormones and ovarian function in patients laparoscopic ovarian cystectomy, but the blood loss is slightly higher.
