累及跟距关节及跟骰关节损伤的跟骨骨折手术治疗效果探讨

Surgical Treatment Effect of Calcaneal Fracture Involving Calcaneal Joint and Calcaneocuboid Joint Injury

目的探究采用手术治疗累及跟距关节及跟骰关节损伤的跟骨骨折的临床疗效。方法选取佳木斯市中心医院收治的累及跟距关节及跟骰关节损伤的跟骨骨折患者41例进行研究,对患者采用手术治疗,手术方法为根骨解剖钢板内固定术,骨外侧做U形切口,统计患者伤口愈合、骨折恢复情况,并判定疗效。结果患者愈合用时为2～4月,切口愈合过程出现并发症的概率为7.32%;术后3个月,患者B?hler角前后比较差异显著,Gissane角治疗前后差异明显,采用Maryland足部评分系统评定患者疗效,优24例,良14例,中3例,差0例,优良率92.68%。结论手术治疗对累及跟距关节及跟骰关节损伤的跟骨骨折治疗效果显著,患者骨折愈合情况良好,具有较好的推广意义。

Objective To explore the clinical effect of surgical treatment of calcaneal fracture involving calcaneal joint and calcaneocuboid joint. Methods 41 cases of calcaneal fracture with calcaneal joint and calcaneocuboid joint injury were studied in the hospital. The patients were treated by operation. The operative method was internal fixation of anatomical plate of root bone and U-shaped incision on the lateral side of bone. The wound healing and fracture recovery were counted,and the curative effect was evaluated. Results The healing time was 2 ~ 4 months months,the probability of complications during incision healing was 7.32%. The mean Bhler angle was significantly higher than 3 months after operation. Difference of Gcissane angle was significant before and after treatment. Maryland foot scoring system was used to evaluate the curative effect of the patients. 24 cases were excellent,14 cases were good,3 cases were middle,and 0 case was poor. The excellent and good rate was 92.68%. Conclusion Surgical treatment is effective for calcaneal fracture with calcaneal and calcaneocuboid joint injury.
