不同肠道准备方案在儿童单气囊小肠镜检查中的应用效果对比

Bowel preparation of pediatric single balloon enteroscopy:a randomized,single-blind,controlled study

目的探讨安全、有效的儿童单气囊小肠镜检查前肠道准备方案。方法遵循随机化原则将湖南省儿童医院接受经肛进镜治疗的97例患儿分成A、B、C三组。A组口服聚乙二醇(Polyethylene glycol,PEG)+西甲硅油;B组口服PEG;C组口服乳果糖+西甲硅油;经上述干预后分别针对三组肠道清洁效果、疾病确诊率、不良反应发生率及耐受率进行对比。结果A组肠道清洁度得分低于B、C两组(P<0.05),但B、C两组间肠道清洁度得分没有统计学差异(P>0.05)。A组疾病确诊率显著高于B、C两组(P<0.05),但B、C两组间疾病确诊率没有统计学差异(P>0.05)。C组不良反应发生率及耐受率高于A、B两组(P<0.05),但A、B两组间不良反应发生率及耐受率无统计学差异(P>0.05)。结论 儿童经肛单气囊小肠镜检查术前肠道准备的过程中,采用PEG联合西甲硅油可达到较为优质的肠道清洁效果,同时有助于提高疾病确诊率并降低不良反应及并发症的发生机率,是儿童单气囊小肠镜经肛检查前肠道准备较为理想的选择。

Objective To explore a kind of safe,effective bowel preparation for children undergoing single balloon enteroscopy(SBE).Methods Retrograde route SBE was performed in 97 cases randomly assigned into A,B,C groups[group A:oral polyethylene glycol(PEG)plus simethicone;group B:oral PEG;group C:oral lactulose plus simethicone].For evaluating bowel cleansing effect,disease diagnostic rate,adverse reactions and complications were compared among groups A,B,C.Results No significant difference existed in intestinal cleanliness between groups B and C of and there were significant differences among groups A and B,C(P<0.05).The cleanliness score of group A was lower than those of groups B&C.Significant differences existed among groups A,B&C(P<0.05).Partition of chi-square method was employed for comparing three groups after treatment.Based upon the level(alpha=0.05),the occurrence of adverse reactions showed no significant difference between groups A and B(P>0.05);The occurrence of complications had statistically significant difference among groups A and C(P<0.05).The incidence of adverse reactions had statistical difference between groups B and C(P<0.05).The rate of compliance was not statistically significant between groups A and B(P>0.05)and statistically significant between groups A and C(P<0.05).Conclusion Conclusion PEG plus oral simethicone are ideal for retrograde route SBE in children.
