摘要
目的应用磁共振波谱定量技术探讨稳定期双相障碍Ⅰ型(BDⅠ)患者的海马代谢改变,以进一步理解双相障碍I型的神经病理生理机制。方法应用3.0 T高场磁共振对25例稳定期BDⅠ型患者以及20名年龄性别匹配的健康对照者的左侧海马行点分辨单体素波谱检查,采用LCModel软件对所获波谱数据进行后处理分析。结果 BDⅠ组与正常对照组海马N-乙酰天冬氨酸(NAA)、胆碱(Cho)、肌酸(Cr),肌醇(MI)、谷氨酸复合物Glx浓度具有统计学差异(P <0.05),其中BDⅠ组海马NAA浓度较对照组减低,Cho、Cr、MI、GLX较对照组升高。结论稳定期双相障碍Ⅰ型患者存在神经元完整性、细胞膜更新、谷氨酸能神经递质等一系列代谢改变。
Objective The aim of this study was to investigate the metabolic alteration of hippocampus in euthymic patients with BD using quantitative MR spectroscopy. Methods proton MRS (1H-MRS)was implemented on 25 euthymic patients and 20 age-and sex-matched healthy controls of left hippocampus using a point-resolved spectroscopy(PRESS)sequence on a 3.0 T MR scanner. Magnetic Resonance Spectroscopy (MRS) data was processed with LCModel software. Results Significant differences were observed for N-acetyl aspartate(NAA), Choline(Cho), Creatine(Cr),Myo-inositol(MI), Glx(Glutamate + Glutamine)(P < 0.05) between 2 groups;Cho、Cr、MI、GLX were significantly increased and NAA were significantly decreased compared with control group. Conclusion Our results demonstrateda wide range of metabolite alterations in Bipolar Ⅰ Euthymia involved in neuronal integrity, membrane turnover and glutamatergic neurotransmission.
