腹腔镜胆囊切除联合不同胆总管探查方法治疗胆总管结石疗效探讨

Laparoscopic cholecystectomy combined with different common bile duct exploration methods for the treatment of common bile duct stones

目的:对比LC联合经胆囊管探查、经胆总管切开探查法治疗胆总管结石的临床疗效。方法:收治胆囊结石合并胆总管结石患者98例,分两组。对照组50例采取LC联合经胆总管切开探查法治疗;观察组48例采取LC联合经胆囊管途径探查法治疗。结果:观察组患者的手术时间、住院时间均明显短于对照组(P<0.05),观察组患者的中转开腹率、并发症率低于对照组,差异有统计学意义(P<0.05)。结论:LC联合经胆囊管探查、经胆总管切开探查法均能够达到治疗目的,但胆囊管探查法创伤更小、安全性更高,应严格把握适应证,制定合理的治疗方案。

Objective:To compare the clinical effect of LC combined with cholecystic duct exploration and choledocholithotomy in the treatment of choledocholithiasis.Methods:98 cases of cholecystolithiasis complicated with choledocholithiasis were selected,they divided into the two groups.50 cases in the control group were treated with LC combined with choledocholithotomy and 48 cases in the observation group were treated by LC combined with cholecystic duct exploration.Results:The operative time and hospitalization time of the patients in the observation group were significantly shorter than those in the control group(P<0.05).The conversion rate and complication rate of patients in the observation group were lower than those in the control group(P<0.05).Conclusion:LC combined with cholecystic duct exploration and choledocholithotomy can achieve the purpose of treatment,but the exploration of cholecystic duct is less traumatic and more safe,so it is necessary to strictly grasp the indications and make a reasonable treatment plan.
