分娩镇痛配合自由体位阴道分娩的临床效果观察

Clinical observation of labor analgesia combined with free position vaginal delivery

目的观察分娩镇痛配合自由体位阴道分娩的临床效果。方法将2017年10月～2018年9月在我院住院行阴道分娩的192例产妇纳入本研究,全部产妇采用随机数字表法随机分为两组,每组各96例,两组均采用分娩镇痛。对照组采取传统平卧位,观察组选择坐位、站位、卧位、蹲位、行走等自由体位,对比两组不同产程所用时间、产后出血量、新生儿窒息发生率。结果观察组产后出血量显著少于对照组(P<0.05);观察组的VAS评分显著低于对照组,差异具有显著性(P<0.05);观察组新生儿窒息发生率虽然低于对照组,但差异无显著性(P>0.05);观察组产妇第一产程、第二产程、第三产程及总产程时间显著短于对照组,差异具有显著性(P<0.05)。结论分娩镇痛配合自由体位阴道分娩可以缩短产程时间,缓解疼痛,减少产后出血量,提高分娩质量,值得临床推广和应用。

Objective To observe the clinical effect of labor analgesia combined with free position vaginal delivery. Methods 192 cases of vaginal delivery in our hospital from October 2017 to September 2018 were included in the study. They were randomly divided into two groups using a random number table method, with 96 cases in each group. Both groups were given labor analgesia, traditional supine position was adopted by the control group, and the observation group was introduced to free positions such as sitting, standing, lying, squatting, and walking. The time spent in different labor stages, the amount of postpartum hemorrhage, and the incidence of neonatal asphyxia were compared. Results The amount of postpartum hemorrhage in the observation group was significantly lower than that in the control group(P<0.05). The VAS score in the observation group was significantly lower than that in the control group(P<0.05). The incidence of neonatal asphyxia in the observation group was lower than that in the control group, but the difference was not significant(P>0.05). The time of the first stage of labor, the second stage of labor, the third stage of labor and the total stage of labor in the observation group were significantly shorter than those of the control group, and the difference was significant(P<0.05). Conclusion Labor analgesia combined with free position vaginal delivery can shorten the time of labor, relieve pain, reduce postpartum hemorrhage, improve the quality of delivery, and is worthy of clinical promotion and application.
