TB-IGRA检测对诊断结核活动的价值探讨

目的探究TB-IGRA(γ-干扰素释放酶联免疫法)检测对诊断结核活动的价值。方法选取2016年3～12月我院收治的可疑结核感染患者108例和同期非结核感染患者130例作为研究对象,其中可疑结核感染患者中结核感染确诊病例为44例,结核感染疑似病例为64例,对受检者TB-IGRA检测,同时行肺部CT、PPD、痰或胸水标本涂片检查,并对相应结果进行追踪随访,最后分析结果。结果 TB-IGRA检测在确诊结核组和疑似结核患者中的阳性率均高于PPD试验的阳性率,在非结核杆菌感染患者阴性率高于PPD试验,差异有统计学意义(P<0.05)。结论 TB-IGRA检测技术的应用有助于早期发现早期诊断结核活动患者,对诊断结核活动具有灵敏度高、特异性好、取材方便、价格合理等优点。

Objective To explore the value of TB-IGRA(gamma-interferon-releasing enzyme-linked immunosorbent assay)for the diagnosis of active tuberculosis. Methods 108 patients with suspected tuberculosis infection and 130 patients without tuberculosis infection treated in our hospital from March to December 2016 were selected. Among the 108 suspected patients, 44 were confirmed, and 64 were suspected. Patients were tested for TB-IGRA, and CT scans of the lungs, PPD tests, and smear examinations of sputum or pleural effusions were performed. The results were followed up and analyzed. Results The positive rates of TB-IGRA test in the confirmed tuberculosis group and suspected tuberculosis group were higher than those in the PPD tests. In patients without tuberculosis infection, the negative rate of TB-IGRA was higher than that of PPD test, and the differences were statistically significant(P<0.05). Conclusion The application of TB-IGRA detection is helpful for early detection and early diagnosis of tuberculosis patients, and it has the advantages of high sensitivity, good specificity, convenient material selection and reasonable price.
