4种固定液对甲状腺冷冻切片固定效果的比较

Comparison of Fixation Effects of Four Fixation Fluids on Frozen Sections of Thyroid Gland

目的探讨不同固定液对甲状腺冷冻切片的固定效果。方法收集福建省立医院南院2017年5月—2018年5月新鲜甲状腺手术切除标本30例,通过徕卡CM1950-1冷冻切片机行快速冷冻切片,分别用4种不同固定液固定,并通过HE染色观察4种固定液对甲状腺冷冻切片的固定效果。结果经10%中性福尔马林固定的冷冻切片,组织结构模糊,核质对比度差,核固缩,核染色模糊,大部分胶质脱落。经95%乙醇固定的冷冻切片染色效果一般。组织结构尚清晰,核质对比度一般,胞核染色不佳,核收缩,滤泡胶质很少着色,部分胶质滤泡缺失。经AF液固定的冷冻切片,组织结构较清晰,核质对比度一般,核轻微收缩,胶质滤泡几乎全部着色。经AFA液固定的冷冻切片,组织结构清晰,颜色鲜艳,细胞核呈蓝色,细胞质呈淡粉色,细胞形态清晰,核质对比度良好,滤泡胶质全部着色。结论 AFA液固定的甲状腺冷冻切片,固定效果好,镜下最接近石蜡切片HE染色,有助于提高诊断的准确性。

Objective To study the effect of different fixation solutions on the frozen sections of thyroid.Methods 30 cases of fresh thyroid operation were collected from May 2017 to May 2018,and the frozen sections were frozen by Leica CM1950-1 cryostat.Four different fixation solutions were used to fix the frozen sections of the thyroid by HE staining.Results The frozen section was fixed by 10%neutral formalin,the structure of the tissue was fuzzy,the contrast of the core was poor,the core was fixed,the nuclei were stained and the most of the colloid was shed.The frozen section was fixed by 95%ethanol.The structure of the tissue is still clear,the nuclear contrast is normal,the nuclear staining is not good,the nucleus is obviously contracted,the colloid is rarely colored,and the partial colloid follicle is missing.The frozen section was fixed by AF liquid,the structure was clear,the contrast of the core was normal,the nucleus was slightly contracted,and the colloid follicle was almost all colored.The frozen section is fixed by the AFA liquid,the structure is clear,the color is bright,the nucleus is blue,the cytoplasm is light pink,the shape of the cells is clear,the nuclear contrast is good,and the follicular colloid is all colored.Conclusion The frozen section of the thyroid,which is fixed by the AFA liquid,has good fixation effect,and the most close to the paraffin section HE staining under the mirror can help to improve the accuracy of the diagnosis.
