血管指数定量化分析对甲状腺功能亢进药物治疗效果的价值 被引量:1

摘要 目的探讨在甲亢药物治疗效果分析上,血管指数定量化的临床价值。方法从我院2016年9月~2018年6月接收的甲亢患者中选取60例作为观察目标,设为病例组,并选取同期甲状腺功能正常志愿者30例为参照组,予以病例组患者甲亢药物治疗,于治疗3个月后,实施血清学检查,并依照血清学指标将其分为甲减组、缓解组、未愈组,同时,在治疗前后对病例组与参照组实施超声检查,对其血管指数予以对比分析;绘制ROC曲线,分析血管指数在甲亢药物治疗效果中的诊断敏感度、特异度。结果 60例病例组患者中,甲减组有23例(38.33%),缓解组有20例(33.33%),未愈组有17例(28.33%),三组临床信息(包括男女比例、年龄、病程)对比,差异无统计学意义(P> 0.05);甲减组、缓解组、未愈组治疗前的血管指数均显著高于参照组,差异有统计学意义(P <0.05);甲减组治疗后的血管指数显著低于参照组,差异有统计学意义(P <0.05);缓解组、未愈组治疗后的血管指数显著高于参照组,差异有统计学意义(P <0.05);甲减组治疗后的血管指数显著低于缓解组、未愈组,差异有统计学意义(P <0.05);缓解组治疗后的血管指数显著低于未愈组,差异有统计学意义(P <0.05)。ROC曲线结果表明:血管指数定量化在甲状腺功能亢进药物治疗中曲线下面积为0.848,其标准差为0.029,用于甲状腺功能亢进患者药物预后评估中具有显著的意义,敏感度与特异度分别为75.3%和81.0%。结论在甲亢药物治疗效果分析上,血管指数能够予以定量化评价,在甲亢诊治以及预后判断上具有重要指导作用。 Objective To explore the clinical value of quantitative analysis of vascularity index in analyzing effect of drug treatment of hyperthyroidism. Methods 60 patients with hyperthyroidism who were admitted and treated in our hospital from September 2016 to June 2018 were selected as research objects and set as the case group. 30 volunteers with normal thyroid function in the same period were selected as the reference group. Patients in the case group were treated with hyperthyroidism drugs. After 3 months of treatment, they were given serological examination and they were divided into the hypothyroidism group, remission group and non-recovery group according to serological indicators. At the same time, ultrasound examination was performed in the case group and the reference group before and after treatment and the vascularity index was compared and analyzed. The ROC curve was drawn to analyze the sensitivity and specificity of the vascularity index in effect of drug treatment of hyperthyroidism. Results Among the 60 patients in the case group, there were 23 cases (38.33%) in the hypothyroidism group, 20 cases (33.33%) in the remission group, and 17 cases (28.33%) in the non-recovery group. There was no statistically significant difference in clinical information (including male and female ratio, age, and course of disease) in the three groups (P>0.05). The vascularity index of hypothyroidism group, remission group and non-recovery group before treatment were all significantly higher than that of the reference group and the difference was statistically significant (P<0.05). The vascularity index after treatment of hypothyroidism group was significantly lower than that of the reference group and the difference was statistically significant (P<0.05). The vascularity index after treatment of remission group and non-recovery group were significantly higher than that of the reference group and the difference was statistically significant (P<0.05). The vascularity index after treatment of hypothyroidism group was significantly lower than that of the remission group and non-recovery group and the difference was statistically significant (P<0.05). The vascularity index after treatment of remission group was significantly lower than that of the non-recovery group and the difference was statistically significant (P<0.05). The ROC curve results showed that the area under the curve and the standard deviation of quantitative analysis of vascularity index in drug treatment of hyperthyroidism were 0.848 and 0.029, which had significant significance in the drug prognosis evaluation of hyperthyroidism patients, with the sensitivity and specificity of 75.3% and 81.0% respectively. Conclusion In analyzing effect of drug treatment of hyperthyroidism, quantitative analysis of vascularity index plays an important guiding role in the diagnosis and prognosis of hyperthyroidism.
