颅脑手术中自体血回输与异体输血的应用研究 被引量:11

摘要 目的探讨自体血回输与异体输血在颅脑手术治疗中的应用及其意义。方法回顾分析2017年10月~2018年3月本院收治的颅内手术患者78例,根据术中采用输血方式的不同分为自体血回输组(A组)和异体输血组(B组)。记录、分析两组术中输血量;术前、术后24h、术后7d患者的Sp O2、RBC、Hb变化及PT、APTT、ALT、AST、TBIL变化;比较两组术后24h至术后7d输血不良反应发生情况。结果两组红细胞输血量比较,差异无统计学意义(P> 0.05);两组术后24h检测RBC、Hb、Sp O2较术前增高,差异有统计学意义(P <0.05)。但两组携氧能力各时段组间比较,差异无统计学意义(P> 0.05);两组术前、术毕24h、术后7d PT、APTT、ALT、AST、TBIL指标组间、组内比较,差异无统计学意义(P> 0.05);A组不良反应发生率低于B组,差异有统计学意义(P <0.05)。结论自体血回输技术安全、有效,携氧功能良好,输血不良反应少,值得临床推广使用。 Objective To explore the application and significance of autologous blood transfusion and allogeneic blood transfusion in craniocerebral surgery. Methods Seventy-eight patients who received craniocerebral surgery in our hospital from October 2017 to March 2018 were retrospectively analyzed and divided into autologous blood transfusion group(Group A)and allogeneic blood transfusion group(Group B)according to the different blood transfusion methods in operation.Intraoperative blood transfusion volume of the two groups was recorded and analyzed.The changes of SpO2,RBC,Hb,PT,APTT,ALT,AST and TBIL in the patients were analyzed before surgery,24h after surgery and 7 days after surgery,and instances of adverse reactions from 24h to 7 days after operation between the two groups were compared. Results There was no significant difference in the amount of red blood cell transfusion between the two groups(P>0.05).The RBC,Hb and SpO2 levels in the two groups 24h after operation were higher than those before the operation,and the difference was statistically significant(P<0.05).However,there was no significant difference in the oxygen carrying capacity between the two groups(P>0.05).There was no significant difference between the two groups in terms of PT,APTT,ALT,AST and TBIL indicators before surgery,24h after surgery and 7 days after surgery(P>0.05).The incidence of adverse reactions in group A was lower than that in group B,and the difference was statistically significant(P<0.05). Conclusion Autologous blood transfusion technology is safe,effective,with good oxygen carrying function,less adverse reactions to transfusion,which is worthy of clinical promotion.
