超声测定患儿解剖参数指导喉罩型号选择 被引量:2

摘要 喉罩是临床小儿麻醉中最为常用的声门上气道装置,其对咽喉部刺激小,置入时血流动力学稳定,提高患儿术后舒适度,同时也方便麻醉科医师对气道的管理。通常根据患儿体重选择使用不同型号的喉罩。然而,以体重为指导选择喉罩的方案不能准确反映患者的体型和头面部情况,尤其是咽喉部容积大小,使其有一定的局限性。选择合适型号的喉罩至关重要,其可保证提供充足的通气,并防止如黏膜损伤,舌后坠和胃胀气等并发症的发生。喉罩的理想大小是通气喉罩能与喉腔周围区域形成一个连续的密封圈,Kim等提供的三维X线气道重建直观显示了喉罩体通气管口与舌骨体基本在同一水平位置,舌骨角接触通气管侧罩体,罩体远端至环状软骨水平。本研究在超声下测定患儿舌骨体至环状软骨弓的距离指导确定喉罩体通气管口至罩体远端长度,测定舌骨角两端的横径指导确定通气管两侧罩体宽度,以此参数作为选择喉罩型号的依据,评价其可行性。
