彩色多普勒超声在筛查痛风性关节炎中的临床意义研究

Study on clinical significance of color Doppler ultrasound in screening for gouty arthritis

目的探讨彩色多普勒超声在筛查痛风性关节炎中的临床意义。方法选取50例痛风性关节炎患者作为试验组,另选取同期50例其他关节炎(类风湿性关节炎16例,骨性关节炎17例,银屑病关节炎17例)患者作为对照组。两组患者均使用彩色多普勒超声对双侧第一跖趾关节进行检查,观察比较两组患者关节积液、滑膜增厚、多点状高回声、双轨征、痛风石检查效果。结果试验组患者共检查100个跖趾关节,关节积液92个(92.00%),滑膜增厚87个(87.00%),多点状高回声71个(71.00%),双轨征61个(61.00%),痛风石75个(75.00%);对照组患者共检查100个跖趾关节,关节积液93个(93.00%),滑膜增厚90个(90.00%),多点状高回声0个,双轨征0个,痛风石0个。两组患者关节积液、滑膜增厚个数比较,差异无统计学意义(P>0.05),试验组患者多点状高回声、双轨征、痛风石个数多于对照组,差异均有统计学意义(P<0.05)。结论应用彩色多普勒超声对痛风性关节炎进行筛查和鉴别,值得临床推广。

Objective To discuss the clinical significance of color Doppler ultrasound in screening for gouty arthritis.Methods There were 50 patients with gouty arthritis as experimental group,and another 50 cases with other arthritis(16 cases of rheumatoid arthritis,17 cases of osteoarthritis,17 cases of psoriatic arthritis)in the same period as control group.The first metatarsophalangeal joint was examined by color Doppler ultrasound in both groups.Observation and comparison were made on joint effusion,synovial membrane thickening,multi-point hyperechoic,double track sign,gout stone examination effect of two groups.Results In the experimental group,100 metatarsophalangeal joints were examined,including 92 joints(92.00%)of joint effusion,87 joints(87.00%)of synovial membrane thickening,71 joints(71.00%)of multi-point hyperechoic,61 joints(61.00%)of double track sign and 75 joints(75.00%)of gout stone.In the control group,100 metatarsophalangeal joints were examined,including 93 joints(93.00%)of joint effusion,90 joints(90.00%)of synovial membrane thickening,0 joint of multi-point hyperechoic,0 joint of double track sign and 0 joint of gout stone.Both groups had no statistically significant difference in cases of joint effusion and synovial membrane thickening(P>0.05).The experimental group had more joints of multi-point hyperechoic,double track sign,gout stone than the control group,and their difference was statistically significant(P<0.05).Conclusion The application of color Doppler ultrasound in the screening and identification of gouty arthritis is worthy of clinical promotion.
