无透视无牵引建立改良前入路在髋关节镜手术中的应用

摘要 目的探讨无透视无牵引建立改良前入路在髋关节镜手术中的应用。方法回顾性分析2016年1月至2018年6月于郑州大学第一附属医院行髋关节镜手术的51例(58髋)患者的临床资料,均在无透视无牵引下建立改良前入路行髋关节镜手术。统计手术入路建立时间、手术操作时间,对比手术前后视觉模拟评分(VAS)及Harris髋关节功能评分。结果手术入路建立时间3~18(5.86±2.52)min,手术操作时间5~31(14.40±5.68)min。术后随访51例,随访3~30个月,VAS疼痛评分术前为(5.79±1.37)分,术后3个月为(1.43±0.70)分(P<0.01);Harris髋关节功能评分术前为(38.89±5.98)分,术后3个月为(86.86±5.50)分(P<0.01)。结论无透视无牵引下建立改良前入路行髋关节镜手术具有手术时间短、并发症少、安全有效等优点,为髋关节镜手术提供了一种新的手术方式。 Objective To investigate the application of improved anterior approach without fluoroscopy and traction in hip arthroscopy.Methods The clinical data of 51 patients(58 hips)who underwent hip arthroscopy in the First Affiliated Hospital of Zhengzhou University from January 2016 to June 2018 were retrospectively analyzed.All portal were established without fluoroscopy and traction.The time of establishing surgical portal and operation were recorded.The visual analogue scale(VAS)score and Harris hip function score were compared before and after operation.Results The time of establishing surgical portal was 3-18(5.86±2.52)min,and the operation time was 5-31(14.40±5.68)min.Fifty-one cases were followed up for 3 to 30 months.The VAS score was 5.79±1.37 before operation and 1.43±0.70 three months after operation(P<0.01);the Harris score was 38.89±5.98 before operation and 86.86±5.50 three months after operation(P<0.01).Conclusions Establishing an improved anterior approach without fluoroscopy and traction for hip arthroscopic surgery has the advantages of saving operation time,reducing complications,great safety and effectiveness.It provides a new operative method for hip arthroscopic surgery.
