个体化饮食营养干预对老年肝癌患者肝动脉化疗栓塞术后营养状况及生活质量的影响

Impact of Individual Diet Intervention on Nutritional Status and Quality of Life of Senile Patients with Hepatocellular Carcinoma after TACE Operation

目的探讨个体化饮食指导对老年肝癌患者肝动脉化疗栓塞术(TACE)营养状况和生活质量的影响,为饮食干预提供依据。方法 2017年3-7月选取本院收治首次接受介入术的原发性肝癌140例患者纳入研究对象。随机将患者分为对照组和干预组,TACE术后1天收集患者数据:体重指数(BMI)、血清白蛋白(ALB)、丙氨酸转氨酶(ALT)、谷草转氨酶(AST),谷氨酰转肽酶(GGT),并完成欧洲营养风险筛查量表(NRS2002)及生活质量量表(EORTC QLQ-C30)。对照组给予常规饮食指导,干预组给予个体化饮食方案。3个月后再次收集患者数据。结果个体化饮食营养干预后,BMI和ALB明显改善(P <0.05);疼痛,食欲丧失、恶心呕吐、便秘、躯体功能、角色功能、情绪功能、社会功能、认知功能等2组对比,P <0.05,差异有统计学意义。结论个体化营养饮食干预对改善老年患者TACE术后营养状况及生活质量,具有重要的临床意义。

Objective To explore the impact of individual diet intervention on nutritional status and quality of life of senile patients with hepatocellular carci noma after transcatheter arterial chemoembolization (TACE) operation, and to provide a basis for dietary intervention. Methods From March to July 2017, 140 patients with primary liver cancer who received interventional therapy for the first time were included in the study. The patients were randomly divided into two groups. The data were collected at first day after TACE: BMI, ALB, ALT, AST, GGT, and completed NRS2002 and EORTC QLQ-C30. The control group was given regular dietary guidance, while the intervention group was given individualized dietary regimen. 3 months later, collected the patient's data again. Results After the individualized diet, BMI and ALB had statistically significant difference (P<0.05);pain loss of appetite, nausea and vomiting, constipation, body function, role function, emotional function, social function, cognitive function, and fatigue had statistically significant difference (P<0.05). Conclusion Individualized nutritional diet intervention has important clinical significance in improving nutritional status and quality of life in elderly patients after TACE.
