双心方治疗冠心病心绞痛合并抑郁状态临床观察

The Clinical Research into Angina Pectoris Combined with Depression Treated with Shuangxin Prescription

目的:观察双心方加减治疗冠心病心绞痛合并抑郁状态的临床疗效。方法:选择在北京中医药大学第三附属医院就诊的符合冠心病心绞痛合并抑郁状态诊断标准的患者50例,随机分为对照组25例和治疗组25例。治疗组因未能坚持服药剔除2例,因观察后期数据未能完整记录剔除1例,对照组因观察数据未能完整记录剔除2例,最终治疗组22例,对照组23例。对照组给予冠心病心绞痛合并抑郁状态的常规基础治疗,并辅以心理引导。治疗组在对照组治疗基础上应用双心方加减治疗。两组均治疗8周。观察两组患者治疗前后心绞痛发作频率、持续时间、治疗有效率、抑郁自评量表评分变化情况。结果:两组治疗后0周、4周、8周心绞痛发作频率及持续时间呈递减模式,治疗后4周、8周与治疗0周比较,差异有统计学意义(P <0. 05);治疗组治疗后4周、8周与对照组同期比较差异有统计学意义(P <0. 05)。对照组有效率为73. 91%,治疗组有效率为90. 90%,两组有效率比较,差异有统计学意义(P <0. 05)。两组治疗后0周、4周、8周抑郁自评量表评分呈递减模式,治疗后4周、8周与治疗0周比较,差异有统计学意义(P <0. 05);治疗组治疗后4周、8周与对照组同期比较,差异有统计学意义(P <0. 05)。两组患者血、尿、便常规及肝、肾功能均未出现明显异常,且无不良反应发生。结论:双心方加减治疗能够减少冠心病心绞痛合并抑郁状态患者的心绞痛发作次数,改善抑郁程度。

Objective:To observe the clinical curative effect of Modified Shuangxin Prescription(MSXP)on angina pectoris combined with depression.Methods:Fifty patients who met the diagnostic criteria of coronary heart disease with depression in the Third Affiliated Hospital of Beijing University of CM were randomly divided into the control group with 25 cases and the treatment group with 25 cases.Then two cases in the treatment group were excluded for failing to adhere to regular medication and one case was excluded for failing to record the data in the later period of observation.In the control group,two cases were excluded because the observation data could not be recorded completely.Finally,in the treatment group,22 cases were observed,while in the control group,23 cases were observed.The control group was given routine basic treatment of coronary atherosclerotic heart disease(CHD)with depression,and assisted with psychological guidance.The treatment group was treated with MSXP granule based on the treatment of the control group.Both groups were treated for 8 weeks.The frequency and duration of angina pectoris attack,the effective rate of angina pectoris and the changes of Self-rating Depression Scale scores were observed before and after the treatment.Results:The frequency and duration of angina pectoris in the 0,4 and 8 weeks of treatment were in a decreasing form.There was significant difference between the 4 and 8 weeks of treatment and the 0 week of treatment(P<0.05).There were significant differences between the treatment group and the control group in the 4 th and 8 th week’s treatment(P<0.05).The effective rate was 73.91%in the control group and 90.90%in the treatment group.There was a significant difference between the two groups(P<0.05).The Self-rating Depression Scale scores for patients in the 0,4 and 8 weeks of treatment were in a decreasing form.There was significant difference between the 4 and 8 weeks of treatment and the 0 week of treatment(P<0.05).There was significant difference between the 4 and 8 weeks of treatment and the 0 week of treatment(P<0.05).There were significant differences between the treatment group and the control group in the 4 th and 8 th week’s treatment(P<0.05).There were no significant abnormalities in the routine urine,liver and kidney functions and no adverse reactions in both groups.Conclusion:MSXP can improve angina attack and depression degree of patients with angina pectoris combined with depression.
