摘要
目的:探讨扶正消瘤汤联合^(125)Ⅰ-放射性粒子植入术对头颈部横纹肌肉瘤患儿术后T淋巴细胞亚群及血清糖链抗原50(carbohydrate antigen 50,CA50)水平变化的影响。方法:将54例头颈部横纹肌肉瘤患儿随机分为对照组与研究组,各27例。入院后给予两组患儿基础放化疗,在此基础上对照组采用^(125)Ⅰ-放射性粒子植入术治疗,研究组采用^(125Ⅰ)-放射性粒子植入术+扶正消瘤汤(持续服用1个月)。结果:两组有效率比较,研究组88.89%高于对照组59.26%,差异有统计学意义(P<0.05);研究组治疗前血清CA50水平与对照组比较,差异无统计学意义(P>0.05),治疗后两组血清CA50水平较治疗前降低,且研究组低于对照组,差异有统计学意义(P<0.05);研究组治疗前CD_3^+、CD_4^+、CD_8^+、CD_4^+/CD_8^+与对照组比较,差异无统计学意义(P>0.05),治疗后研究组CD_3^+、CD_4^+、CD_4^+/CD_8^+高于对照组,CD_8^+低于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,研究组11.11%低于对照组18.52%,但差异无统计学意义(P>0.05);治疗后12个月、18个月、24个月,研究组生存率为92.59%、85.19%、81.48%,高于对照组的88.89%、77.77%、74.07%,但差异无统计学意义(P>0.05)。结论:扶正消瘤汤联合^(125)Ⅰ-放射性粒子植入术治疗儿童头颈部横纹肌肉瘤效果显著,可有效改善患儿T淋巴细胞亚群及血清CA50水平,增强其免疫功能,提高治疗效果及生存率,且不会增加不良反应发生风险,安全性高。
Objective:To investigate the effects of Fuzheng Xiaoliu Decoction combined with 125 I-radioactive seed implantation on the changes of T lymphocyte subsets and serum carbohydrate antigen 50(CA50) levels in children with head and neck rhabdomyosarcoma.Methods:54 children with head and neck rhabdomyosarcoma were randomly divided into the control group and the study group,27 cases each.After admission,the two groups were given basic radiotherapy and chemotherapy.On the basis of this,the control group was treated with 125Ⅰ-radioactive particle implantation.The study group was treated with 125Ⅰ-radioactive particle implantation + Fuzheng Xiaoliu Decoction(continuous use for 1 month).Results:The effective rate of treatment was compared between the two groups.The study group was 88.89% higher than the control group(59.26%,the difference was statistically significant(P<0.05).There was no significant difference in the serum CA50 level between the study group and the control group before treatment(P>0.05),serum CA50 levels in the two groups were lower than those before treatment,and the study group was lower than the control group,the difference was statistically significant(P<0.05);the study group before treatment CD3+,CD4+,CD8+,CD4+/CD8+ and control There was no significant difference between the two groups(P>0.05).After treatment,the CD3+,CD4+,CD4+/CD8+ in the study group was higher than the control group,and the CD8+ was lower than the control group.The difference was statistically significant(P<0.05).The incidence of reaction was 11.11% in the study group was 18.52% lower than that in the control group,but the difference was not statistically significant(P>0.05).The survival rate of the study group was 92.59%,85.19 at 12 months,18 months and 24 months after treatment.%,81.48%,higher than the control group of 88.89%,77.77%,74.07%,but the difference was not statistically significant(P>0.05).Conclusion:Fuzheng Xiaoliu Decoction combined with 125Ⅰ-radioactive particle implantation is effective in treating children′s head and neck rhabdomyosarcoma,which can effectively improve the T lymphocyte subsets and serum CA50 levels,enhance their immune function,and improve the therapeutic effect and survival rate.It does not increase the risk of adverse reactions so it is safe.
