自动乳腺全容积成像与超声弹性成像对乳腺肿块的诊断价值比较 被引量:10

Comparison of diagnostic value of automated breast volume scanner and ultrasonic elastography in breast mass

摘要 目的比较自动乳腺全容积成像技术(ABVS)与超声弹性成像技术(UE)对乳腺肿块诊断的敏感度、准确性和特异性。方法选取2016年5月至2017年7月在解放军第二六六医院接受外科手术治疗的乳腺肿块患者132例,采用ABVS、UE、二维超声技术进行诊断,与术后病理检测结果进行对比。结果二维超声诊断敏感度为76.54%(62/81),准确性为75.0%(114/152),特异性为73.24%(52/71);UE诊断敏感度为80.25%(65/81),准确性为78.95%(120/152),特异性为77.46%(55/71);ABVS诊断敏感度为88.89%(72/81),准确性为88.16%(134/152),特异性为87.32%(62/71)。ABVS诊断敏感度优于二维超声(χ^2=4.318,P<0.05),但与UE差异无统计学意义(χ^2=2.318,P>0.05);ABVS诊断准确性和特异性显著优于UE和二维超声(准确性:χ1^2=8.756,χ2^2=4.692;特异性:χ1^2=4.449,χ2^2=4.135;均P<0.05)。结论ABVS诊断乳腺病变具有较好的敏感度、准确性和特异性,临床超声科室医生可在乳腺肿块诊断中积极采用ABVS技术,以提高乳腺癌的检出率,提高患者的治愈率和生存率。 Objective To compare the sensitivity,accuracy and specificity between automated breast volume scanner(ABVS)and ultrasonic elastography(UE)in the diagnosis of breast mass.MethodsFrom May 2016 to July 2017,132 patients with breast mass in the No.266 Hospital of PLA were chosen in the research.ABVS,UE and two-dimentional ultrasonography were used for diagnosis,and the diagnostic results were compared with pathological results.ResultsThe sensitivity,accuracy and the specificity of two-dimensional ultrasonography were 76.54%(62/81),75.0%(114/152),73.24%(52/71),respectively,which of UE were 80.25%(65/81),78.95%(120/152)and 77.46%(55/71),respectively,which of ABVS were 88.89%(72/81),88.16%(134/152)and 87.32%(62/71),respectively.The sensitivity of ABVS was higher than that of two-dimensional ultrasonography(χ^2=4.318,P<0.05),while had no statistically significant difference compared with that of UE(χ^2=2.318,P>0.05).The accuracy and specificity of ABVS were higher than those of UE and two-dimensional ultrasonography(accuracy:χ1^2=8.756,χ2^2=4.692;specificity:χ1^2=4.449,χ2^2=4.135;all P<0.05).ConclusionThe sensitivity,accuracy and specificity of ABVS in the diagnosis of breast lesions are high,and doctors from ultrasonic department are recommended to use ABVS to diagnose breast cancer.This can improve the detectable rate of breast cancer and the cure rate and survival rate of patients.
