TACE联合全身化疗治疗结直肠癌肝转移的临床疗效

TACE Combined with General Chemotherapy for Liver Metastasis of Colorectal Carcinoma:Clinical Effect

为研究经导管动脉化疗栓塞术(TACE)联合全身化疗治疗结直肠癌肝转移患者的临床疗效,回顾性分析2004年1月至2012年6月我院行TACE联合全身化疗(40例,研究组)和单纯接受全身化疗(40例,对照组)的结直肠癌肝转移患者的临床资料。观察比较2组患者治疗前后血清丙氨酸氨基转移酶(ALT)水平,治疗后并发症和不良反应发生率,生存时间及生存率。结果显示,2组治疗前及治疗后1周血清ALT水平比较,差异均无统计学意义(P>0.05)。研究组治疗后1年、3年和5年生存率分别为92.5%、32.5%和7.5%,高于对照组的85.0%、10.0%和2.5%(P <0.05);研究组的中位生存时间为30.5个月,高于对照组的17.5个月(P <0.05)。2组均未发生与治疗相关的严重并发症,2组间不良反应发生率比较,差异无统计学意义。结果表明,与单纯全身化疗相比,TACE联合全身化疗能提高结直肠癌肝转移的疗效,且不增加治疗的毒副反应。

In order to investigate the clinical effect transcatheter arterial chemoembolization(TACE) combined with general chemotherapy for liver metastasis of colorectal carcinoma,authors retrospectively analysed the data including research group(40 cases) and control group(40 cases) who had received surgical treatment in author's hospital(2014-01-2012-06): in research group,above combination chemotherapy;in control group,alone general chemotherapy,observed and compared both groups' serum ALT level before and after treatment,the incidence of postoperative complication and adverse reaction,and survival duration,as well as survival rate.As results,in serum ALT level before treatment and 1 week after treatment there was no statistical difference between the two groups( P>0.05);in postoperative 1,3 and 5-year survival rate research group was significantly higher than control group(92.5% vs 85.0%,32.5% vs 10.0%,7.5%vs 2.5%,P<0.05),and in median survival time also higher(30.5 vs 17.5 months,P<0.05);in the untoward reaction incidence there was no statistical difference between the two groups( P>0.05);in both groups no severe complication related to treatment occured.Results show that TACE combined with general chemotherapy has better effect,and without more untoward reaction than alone chemotherapy,in the treatment of liver metastasis from colorectal carcinoma.
