依达拉奉联合高压氧治疗对急性脑出血患者血浆β-内啡肽GnRHN端脑钠肽前体和肽素表达的影响 被引量:3

摘要 目的探讨依达拉奉联合高压氧对急性脑出血患者血浆β-内啡肽、促性腺激素释放激素、N端脑钠肽前体、和肽素表达的影响。方法将96例急性脑出血患者按随机数字表法分为观察组与对照组,每组48例,在常规治疗基础上均予以依达拉奉治疗,观察组联合高压氧治疗,观察20d。治疗前后抽取空腹静脉血检测两组血浆β-内啡肽、促性腺激素释放激素、N端脑钠肽前体、和肽素水平,CT检查计算脑水肿体积,采用美国国立卫生研究院卒中量表评定神经功能缺损程度,采用Barthel指数评定日常生活能力,比较两组临床疗效。结果治疗后两组血浆β-内啡肽、促性腺激素释放激素、N端脑钠肽前体、和肽素表达及美国国立卫生研究院卒中量表评分均较治疗前显著降低(P<0.05),且观察组显著低于(P<0.01);两组脑水肿体积均较治疗前显著缩小(P<0.01),且观察组脑水肿体积显著小于对照组(P<0.01);两组Barthel指数均较治疗前显著升高(P<0.01),且观察组Barthel指数显著高于对照组(P<0.01);观察组总有效率(89.58%)显著高于对照组(70.83%)(P<0.05)。结论依达拉奉联合高压氧治疗急性脑出血患者具有协同增效作用,临床疗效显著,能有效降低血浆β内啡肽、促性腺激素释放激素、N端脑钠肽前体及和肽素的表达,缓解脑水肿和神经缺损程度,对改善患者的日常生活能力具有积极意义。 Objective To study effects of edaravone combined with hyperbaric oxygen on plasma B-endorphin(-EP),gonadotropin releasing hormone(GnRH),N-terminal pro-brain natriuretic peptide(NTproBNP)and copeptin in patients with acute intracerebral hemorrhage.Methods 96 patients of acute intracerebral hemorrhage were randomly divided into the observation group and the control group,48 cases in each group.On the basis of conventional treatment,the control group was treated with edaravone,while the observation group was combined with hyperbaric oxygen,all patients were observed for 20 d.Fasting venous blood was taken before and after treatment to detect plasma-EP,GnRH,NT-proBNP,copeptin levels.CT examination was used to calculate the brain edema volume.NIISS was used to assess the degree of neurological deficit,and Barthel index was used to assess the ability of daily living.The clinical efficacy of the two groups was compared.Results After treatment,the plasmaβ-EP,GnRH,NTproBNP,copeptin and NIHSS score in the two groups were significantly lower than those before treatment(P<0.05),and the plasma 8-EP,GnRH,NT-proBNP,copeptin,and NIHSS score in the observation group were significantly lower than those in the control group(P<0.05);the volume of brain edema in the two groups were significantly lower than those before treatment(P<0.05),and the volume of brain edema in the observation group was significantly smaller than that in the control group(P<0.05);the Barthel index of the two groups were significantly higher than those before treatment(P<0.05),the Barthel index in the observation group was significantly higher than that in the control group(P<0.05);the total effective rate of clinical.efficacy in observation group(89.58%)was higher than that in control group(70.83%)(P<0.05).Conclusion Edaravone combined with hyperbaric oxygen therapy for patients with acute cerebral hemorrhage has synergistic effect,which can effectively decrease the expression of plasmaβ-EP,GnRH,NT-proBNP and copeptin,and can relieve the volume of brain edema and the degree of nerve defect,it has positive significance for improving patients'daily living ability.
