摘要
目的研究基于员工援助计划模式的医务工作者压力管理模型构建,为缓解医务工作者工作压力、提高心身健康水平及工作效率提供参考.方法采用员工援助计划混合模式建立本院医务工作者压力管理,成立院内员工援助计划管理小组,在外设员工援助计划专业机构的协助下,引入计划-执行-检查-纠正质量管理环,每3个月按照压力预防-评估-控制-善后4个方面内容对医务工作者进行压力管理,若有特殊或重大人员变更,启动压力评估步骤对医务工作者实施压力管理;按照计划-执行-检查-纠正质量管理环操作模式,持续改进完善员工援助计划压力管理模型.将员工援助计划模式实施前1 a的医务工作者设为对照组(n=1732),实施后1a的医务工作者设为观察组(n=1732);采用临床医生压力源量表、护士压力源量表评定两组医务工作者的工作压力源状况,采用焦虑自评量表、抑郁自评量表评定两组医务工作者的焦虑抑郁状况;统计两组医务工作者与对应患者发生医患、护患纠纷情况,计算医患、护患纠纷率,并进行对比分析.结果观察组临床医生压力源量表、护士压力源量表、焦虑自评量表、抑郁自评量表评分均显著低于对照组(P<0.01);医患、护患纠纷率显著低于对照组(P<0.01).结论构建基于员工援助计划混合模式的医务工作者压力管理模型,有利于缓解医务工作者的工作压力,提高心身健康水平,对促进医务工作者的管理质量和水平具有较高价值.
Objective To study the construction of the stress management model of medical workers based on the employee assistance program(EAP)mode and provide a reference for relieving the pressure on medical workers,improving their mental health and work efficiency.Methods Establish the pressure management of medical workers in our hospital by using EAP mixed mode and the in-house EAP management team.Introduce the plan-do-check-adjust(PDCA)quality management loop with the assistance of the EAP.Pressure management of medical workers was conducted in four aspects of stress prevention-evaluation-control-aftercare every 3 months,and if there were special or major personnel changes,start the stress assessment step to implement pressure management for medical workers.According to the PDCA quality management loop operation mode,continuously improve the employee assistance plan stress management model.The medical workers I year before the implementation of the EAP model were set as the control group(n=1732),and the medical workers 1 year after the implementation were set as the obser-vation group(n=1 732).Using the clinician pressure source scale and NJSI to assess the status of the work stressors of the two groups of medical workers.Anxicty and depression status of two groups of medical workers were assessed by SAS and SDS.Analyze the disputes between doctors and patients and nurses and patients in the two groups,calculate the rate of disputes between doctors and patients,nurses and patients,and conduct comparative analysis.Results The clinician pressure source scale,NJSI,SAS and SDS scores in the observation group were significantly lower than those of the control group(P<0.01);the rate of disputes between doctors and patients,nurses and patients was significantly lower than that of the control group(P<0.01).Conclusion The construction of stress management model for medical workers based on the EAP model is conducive to alleviating the work pressure of medical workers and improving the physical and mental health,and has a high value for promoting the quality and level of management of medical workers.
