手术联合赝复体式放射性粒子植入治疗腭部腺源性恶性肿瘤 被引量:4

Surgery combined with prosthetic radioactive particle implantation for the treatment of palatal adenogenic malignant tumor

摘要 目的探讨手术联合贋复体式放射性粒子植入治疗腭部腺源性恶性肿瘤的临床应用价值。方法选取2012年5月一2015年6月就诊于徐州市中心医院确诊为腭部腺源性恶性肿瘤患者18例,均在行上颌骨次全切手术后,将患者术前CT导入治疗计划系统(TPS)制定放疗计划,并制作贋复体布源器。术后定期随访,观察疗效及并发症结果18例患者术后验证D90和V100均满足治疗需要.贋复体布源器周围重要器官均在耐受剂量以内.截至末次随访日期,总有效率为88.9%,总生存率为83.3%,无4级放疗反应。结论腭部腺源性恶性肿瘤术后联合矚复体布源器,可以在修复颌骨缺损的同时,减少恶性肿瘤的复发和转移。 Objective To investigate the clinical value of surgery combined with pseudo-compound radioactive particle implantation in the treatment of palatal adenogenic malignant tumor. Methods A total of 18 patients with palatal adenogenic malignant tumors diagnosed in Xuzhou Central Hospital from May 2012 to June 2015 were selected and all of them underwent subtotal maxillary resection. The preoperative CT was introduced into the treatment planning system (TPS) to formulate the radiotherapy plan, and a prosthetic implant was made. Post-operative follow up was performed regularly to observe the efficacy and complications. Results Postoperatively, it was showed that 1)90 and V 100 met the needs of treatment in all the 18 patients, and the important organs around the prosthetic implant were within the tolerant dose. At the last follow-up date, the total effective rate was 88. 9%, the overall survival rate was 83. 3%, and there was no grade 4 radiotherapy response. Conclusions The combination of surgery and prosthetic implant can reduce the recunence cind metastasis of palatal adenogenic malignant tumor while repairing the jaw defect.
