阻塞性睡眠呼吸暂停综合征与早期动脉粥样硬化的相关性研究

Correlation analysis between obstructive sleep apnea syndrome and early atherosclerosis

目的探讨阻塞性睡眠呼吸暂停综合征(OSAS)与早期动脉粥样硬化的相关性。方法选取OSAS患者200例,根据睡眠呼吸暂停低通气指数AHI的高低将OSAS患者分为3组:轻度OSAS组(n=87)、中度OSAS(n=62)组和重度OSAS(n=51)组,另选取同期健康体检者180例作为对照组,对入选者进行问卷调查、体格检查、血脂、血糖、尿酸等相关指标的测定。结果与对照相比,OSAS组患者糖尿病病史比率、高血压病史比率、收缩压、舒张压、BMI、TG、尿酸、FBG和CIMT值均升高,而HDL-C降低,差异均有统计学意义(P<0.05);重度OSAS组CIMT值高于中度组和轻度组(1.28±0.28 vs 1.08±0.24 vs 0.94±0.19,P<0.05);Logistic回归分析提示校正一些危险因素如血脂、血糖后OSAS仍然是CIMT增厚的独立危险因素。结论 OSAS患者CIMT值增高,OSAS可能是早期动脉粥样硬化的独立危险因素。

Objective To investigate the relationship between obstructive sleep apnea syndrome(OSAS) and early atherosclerosis. Methods A total of 200 patients with OSAS were collected and divided into the mild OSAS group(n=87), the moderate group(n=62) and the severe OSAS group(n=51). 180 healthy people were enrolled into our study as the control group. Anthropometric and biochemical examinations were determined in all subjects. Results Compared with the control group, patients with OSAS had higher diabetes ratio, hypertension ratio, systolic pressure, diastolic pressure, BMI, TG, uric acid, FBG and CIMT, while they had lower HDL-C, with the differences statistically significant(P<0.05). Patients with severe OSAS had higher CIMT than patients with mild and moderate OSAS(1.28±0.28 vs 1.08±0.24 vs 0.94±0.19, P<0.05). Logistic analysis showed that OSAS was an independent risk factor of CIMT after adjustment for lipid profile, FBG. Conclusion CIMT increased in patients with OSAS, which may be an independent risk factor of early atherosclerosis.
