保乳术和改良的乳腺癌根治术治疗乳腺癌的临床对照研究

A clinical controlled study of breast-conserving surgery and modified radical mastectomy in treatment of breast cancer

目的探究保乳术及改良的乳腺癌根治术治疗乳腺癌的临床疗效。方法收集45例乳腺癌患者进行研究,均收治于本院2016年11月～2017年6月,采用随机数字表法分为两组研究,实验组纳入23例,采用保乳术治疗,对照组纳入22例,采用改良乳腺根治术治疗,观察两组各手术指标并进行生活质量评分。结果经保乳术治疗的实验组,切口长度、手术时间、住院时间均短于改良乳腺癌根治术的对照组,术中出血量及引流量也均少于对照组,差异有统计学意义(P <0.05);实验组经保乳术治疗,躯体运动能力、心理健康状态、精神状态及社会功能评分均高于对照组,差异有统计学意义(P <0.05)。术后进行1年的随访,实验组无复发病例,对照组仅1例复发,复发率为4.5%,两组复发率均较低,差异无统计学意义(P> 0.05)。结论乳腺癌患者采用保乳术治疗,保留了乳房基本形态,可达到显著的美容效果,同时缩短了病情恢复时间,加速了疾病康复进程,值得采用。

Objective To explore the clinical curative effects of breast-conserving surgery and modified radical mastectomy in treatment of breast cancer.Methods Forty-five patients with breast cancer who were admitted and treated in our hospital from November 2016 to June 2017 were selected.They were divided into two groups according to random number table method.23 patients in the experimental group were treated with breast conserving surgery while 22 patients in the control group were treated with modified radical mastectomy.The surgical indexes of the two groups were observed and the quality of life was scored.Results The incision length,operation time and hospital stay of the experimental group treated with breast conserving surgery were shorter than those of the control group treated with modified radical mastectomy.Intraoperative blood loss and drainage volume of the experimental group were also less than the control group.All the comparisons were statistically significant(P<0.05).After breast-conserving surgery,the scores of physical movement ability,mental health status,mental state and social function in the experimental group were all higher than those in the control group.The comparison had statistical significance(P<0.05).After 1 year of follow-up,there was no recurrence in the experimental group and only 1 recurrence in the control group,with a recurrence rate of 4.5%.The recurrence rates of both groups were low,and the comparison was not statistically significant(P>0.05).Conclusion Breast-conserving surgery for patients with breast cancer can retain the basic shape of the breast,which achieves significant cosmetic effects.In addition,it can shorten the recovery time of the disease and accelerate the process of rehabilitation,which is worthy of application.
