针刺加颈椎矫正配合桂枝加葛根汤治疗寰枢关节错位的临床研究

Clinical Effect of Acupuncture Combined with Cervical Vertebra Correction Combined with Guizhi and Gegen Decoction in Treatment of Atlantoaxial Dislocation

目的:研究针刺加颈椎矫正配合桂枝加葛根汤治疗寰枢关节错位的临床疗效。方法:选取从2015年3月—2017年3月本院收治的寰枢关节错位患者60例作为研究对象,采用随机数字表法将60例患者随机均分为两组。两组均采取中医常规颈椎矫正手法复位治疗;观察组给予针刺加颈椎矫正配合桂枝加葛根汤。观察两组患者治疗后的临床疗效,比较两组患者治疗前后颈性弦晕症状与功能评估量表积分变化及寰枢关节指标情况。结果:观察组总有效率93. 33%明显大于对照组总有效率73. 33%,观察组临床疗效明显优于对照组,差异有统计学意义(P <0. 05)。两组治疗后组间比较,观察组眩晕、头颈痛、生活心理适应及总积分均明显高于对照组,差异有统计学意义(P <0. 05)。经治疗两组患者齿突寰椎轴线差值及齿突侧块间距差值显著降低,观察组齿突寰椎轴线差值及齿突侧块间距差值明显低于对照组,差异有统计学意义(P <0. 05)。结论:针刺加颈椎矫正配合桂枝加葛根汤治疗寰枢关节错位疗效显著,能有效改善患者临床症状,值得临床推广。

Objective: To study the clinical effect of acupuncture combined with cervical vertebra correction combined with Guizhi and Gegen Decoction in the treatment of atlantoaxial dislocation. Methods: From March 2015 to March 2017,60 patients with atlantoaxial dislocation were selected as the study subjects,and 60 patients were randomly divided into two groups. The two groups were treated with traditional Chinese medicine cervical reduction and manipulative reduction. The observation group was treated with acupuncture and cervical vertebra correction,combined with Guizhi and Gegen Decoction. The clinical efficacy of two groups after treatment was observed. The cervical chord symptoms and functional evaluation scales and the index of atlantoaxial joint were compared between the two groups before and after treatment. Results: The total effective rate of the observation group was93. 33%,significantly higher than that of the control group( 73. 33%),and the clinical efficacy of the observation group was significantly better than that of the control group. The difference was statistically significant( P < 0. 05). Compared with the control group,the dizziness,head and neck pain,life psychological adaptation and total scores of the two groups were significantly higher than those of the control group and the difference was statistically significant( P < 0. 05). After treatment,two groups’ odontoid atlas and axis between odontoid process and lateral spacing between the difference was significantly decreased. The observation group’s atlas odontoid axis difference and odontoid process and lateral spacing between the difference was significantly lower than those of the control group and the difference was statistically significant( P < 0. 05). Conclusion: The effect of acupuncture combined with cervical vertebra correction combined with Guizhi and Gegen Decoction in treatment of atlantoaxial joint dislocation is remarkable. It can effectively improve the clinical symptoms of patients and is worthy of clinical promotion.
