锡类散联合龙珠软膏治疗慢性肛裂切扩术后近远期疗效及对疼痛影响

Short term and Long-term Curative Effects of Xilei Powder Combined with Longzhu Ointment in the Treatment of Chronic anal Fissure after Anal Fissure Cutting and Its Influence on Pain

目的:探讨锡类散联合龙珠软膏治疗慢性肛裂切扩术后的近远期疗效及对疼痛的影响。方法:选取我院收治的慢性肛裂患者60例,采用随机数字表法将其分为观察组和对照组,各30例。2组患者均行肛裂切扩术且术后进行常规处理,对照组涂抹龙珠软膏于创面,观察组同时将锡类散喷撒于创面上,2周为1个疗程,1个疗程结束后统计两组临床疗效,比较两组治疗后创面水肿、疼痛及肉芽增生恢复情况及创面愈合时间,术后随访6个月,统计复发情况。结果:1个疗程结束后,观察组总有效率为90.00%,明显高于对照组的73.33%(P<0.05)。随访期间观察组和对照组均各有1例(3.33%)复发,两组复发率差异无统计学意义(P>0.05)。术后3～15 d,两组创面VAS评分、水肿评分及肉芽增生评分逐渐降低(P<0.01),且观察组各项评分均低于对照组(P<0.05或P<0.01)。与术前比较,术后7d两组创面面积均明显缩小,且观察组小于对照组(P<0.01),观察组愈合时间明显短于对照组(P<0.05)。结论:锡类散联合龙珠软膏可加速慢性肛裂切扩术后患者创面的恢复,减轻疼痛程度,近期疗效显著,但远期疗效与单用龙珠软膏无明显差异。

Objective:To study the short term and long-term curative effects of Xilei Powder combined with Longzhu Ointment in the treatment of chronic anal fissure after anal fissure cutting and its influence on pain. Methods:60 cases of patients with chronic anal fissure in our hospital were selected and divided into the observation group and the control group according to the random digital table method,30 cases in each group. Patients of the two groups all underwent anal fissure cutting and postoperative routine treatment. The control group smeared Longzhu Ointment on the wound,the observation group sprinkled Xilei Powder on the wound at the same time,and two weeks was 1 course of treatment. After 1 course of treatment,the clinical effect in the two group was statistically analyzed,and the recovery of wound edema,pain and granulosis and the healing time of wound healing after treatment were compared between the two groups. The followup period was 6 months,and the recurrence rate was statistically analyzed. Results:The total effective rate after 1 course of treatment of the observation group was 90.00%,which was significantly higher than 73.33%of the control group(P<0.05). During the follow-up period,1 case(3.33%)relapsed in the observation group and the control group,and there was no significant difference in recurrence rate between the two groups(P>0.05). 3-15 d after operation,the VAS score,edema score and granulation score of the two groups gradually decreased(P<0.01),and the scores of the observation group were lower than those of the control group(P<0.05 or P<0.01). Compared with before operation,7 d after operation wound surface area of the two groups significantly reduced,and the observation group was less than the control group(P<0.01),and the healing time of the observation group was significantly shorter than that of the control group(P<0.05). Conclusion:Xilei Powder combined with Longzhu Ointment can accelerate wound healing and relieve pain after anal fissure cutting,short term curative effects is significant,however,there was no significant difference in long-term curative effects with Longzhu Ointment alone.
