超声内镜在早期胃癌术前分期中的应用效果观察

Observation on Application Effect of Endoscopic Ultrasonography in Preoperative Staging of Early Gastric Cancer

目的探讨超声内镜在早期胃癌术前分期中的应用效果。方法选取2016年1月至2018年11月我院收治的早期胃癌患者60例,均行超声内镜检查,并以病理检查结果为标准,分析超声内镜在早期胃癌术前分期中的效果。结果超声内镜诊断早期胃癌T分期结果与病理检查结果的一致性较高,差异有统计学意义(κ=0.668, P=0.000)。超声内镜检查Tis期、 T1a期、T1b期的检出率分别为83.33%、 80.95%、 78.79%。超声内镜诊断早期胃癌N0期、 N1期、 N2期、 N3期的检出率分别为30.43%、 47.06%、 45.45%、 55.56%。超声内镜诊断早期胃癌术前M分期的准确性为55.00%。结论超声内镜在早期胃癌术前T分期诊断中具有较高的诊断价值,但在N分期、 M分期诊断中存在不足。

Objective To explore the application effect of endoscopic ultrasonography in preoperative staging of early gastric cancer.Methods 60 cases of patients with early gastric cancer admitted to our hospital from January 2016 to November 2018 were selected adn given endoscopic ultrasonography. The effect of endoscopic ultrasonography in preoperative staging of early gastric cancer was analyzed based on the pathological results. Results The T-staging results of endoscopic ultrasonography in the diagnosis of early gastric cancer were had higher consistency with those of pathological examination(κ=0.668, P=0.000). The detection rates of Tis stage, T1a stage and T1b stage with endoscopic ultrasonography were respectively 83.33%, 80.95% and 78.79%. The detection rates of N0 stage, N1 stage, N2 stage,N3 stage in the diagnosis of early gastric cancer by endoscopic ultrasonography were respectively 30.43%, 47.06%, 45.45% and 55.56%.The accuracy of endoscopic ultrasonography in the diagnosis of early gastric cancer M stage was 55.00%. Conclusions Endoscopic ultrasonography has higher diagnostic value in preoperative T-staging of early gastric cancer, but it has deficiencies in N-staging and M-staging diagnosis.
