2型糖尿病患者高尿酸与代谢综合征及血管并发症的相关性 被引量:7

Relationship Between Hyperuricemia and Metabolic Syndrome and Vascular Complications in Patients with Type 2 Diabetes

摘要 目的:观察分析2型糖尿病患者高尿酸与代谢综合征及血管并发症的关系。方法:选取笔者所在医院2016年1月-2018年1月收治的200例2型糖尿病患者,按照不同血尿酸水平分为正常血尿酸水平组(100例)和高血尿酸水平组(100例)。分析两组2型糖尿病患者的代谢综合征、血管并发症发生率,采用Pearson相关性分析尿酸与HbA1c、FBG、BMI、TG、TC、LDL-C及HDL-C的相关性,并采用多因素Logistic回归分析尿酸水平与血管并发症的关系。结果:两组2型糖尿病患者在空腹血糖(FBG)比较,差异无统计学意义(P>0.05),高血尿酸水平组2型糖尿病患者的糖化血红蛋白(HbA1c)、体重指数(BMI)、三酰甘油(TG)、胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)等指标比较显著高于正常血尿酸水平组(P<0.05),而HDL-C(高密度脂蛋白胆固醇)水平显著低于正常血尿酸水平组(P<0.05)。Pearson相关性分析显示,血尿酸水平与BMI、TG、LDL-C及HbA1c呈正相关(P<0.05),与HDL-C呈负相关(P<0.05)。高血尿酸水平组2型糖尿病患者的糖尿病下肢血管病变、脑梗死、冠心病、糖尿病肾病和糖尿病视网膜病变等并发症发生率显著高于正常血尿酸水平组(P<0.05),多因素Logistic回归分析显示,高血尿酸水平是2型糖尿病患者的糖尿病下肢血管病变、脑梗死、冠心病、糖尿病肾病和糖尿病视网膜病变的危险因素。结论:2型糖尿病患者的高尿酸水平是引起血管并发症和代谢综合征的重要危险因素。 Objective:To observe the relationship between high uric acid and metabolic syndrome and vascular complications in patients with type 2 diabetes mellitus.Method:200 patients with type 2 diabetes admitted to our hospital from January 2016 to January 2016 were divided into normal blood uric acid level group(100 cases) and high blood uric acid level group(100 cases) according to different blood uric acid levels.The incidence of metabolic syndrome and vascular complications in patients with type 2 diabetes were analyzed.The relationship between uric acid level and HbA1c,FBG,BMI,TG,TC,LDL-C,HDL-C were analyzed with Pearson correlation analys,the relationship between uric acid level and vascular complications were analyzed with multivariate Logistic regression analysis.Result:The fasting blood glucose(FBG) of two groups were compared,there was no significant difference(P>0.05).The glycosylated hemoglobin(HbA1c), body mass index(BMI),acylglycerol(TG),cholesterol(TC) and low-density lipoprotein cholesterol(LDL-C) of high blood uric acid level group were significantly higher than those of normal blood uric acid level(P<0.05),while high-density lipoprotein cholesterol(HDL-C) level was significantly lower than that of normal serum uric acid level group(P<0.05).The Pearson correlation analys showed that blood uric acid level were positively correlated with BMI,TG,LDL-C and HbA1c(P<0.05),and negatively correlated with HDL-C(P<0.05).The incidence of complications such as diabetic lower extremity vascular disease,cerebral infarction,coronary heart disease,diabetic nephropathy and diabetic retinopathy in patients with type 2 diabetes mellitus was significantly higher than that in normal blood uric acid level group(P<0.05),and the multivariate Logistic regression analysis showed the high uric acid were the risk factors of diabetic lower extremity vascular disease,cerebral infarction,coronary heart disease,diabetic nephropathy and diabetic retinopathy of patients with type 2 diabetes mellitus. Conclusion:High uric acid levels in patients with type 2 diabetes are important risk factors for vascular complications and metabolic syndrome.
