摘要 本文介绍了根本原因分析(RCA)方法的重要性、必要性及工作流程。以某核电站设备运行期间插件指示灯出现闪亮/不亮事件为例,按照RCA工作流程,使用头脑风暴法,分析得到了指示灯闪亮/不亮事件的可能故障模式及相关证据,初步确定出了造成事件后果的可能性原因。使用无替换定时截尾试验方法,计算出了指示灯的平均寿命不满足插件设计寿命的要求,确定出指示灯自身质量缺陷是造成系统插件指示灯闪亮/不亮事件的根本原因。制定了纠正行动,采取了纠正措施,建立了经验反馈体系,从根本原因上解决了系统插件指示灯闪亮/不亮的问题。通过效果跟踪评价,验证了RCA方法应用的正确性。 This paper describes the importance, necessary and the workflow of the RCA. A case study of the event of the flash/dark of the indicator in the modules of the nuclear power plant,relevant failure modes and possible evidences of the indicator of modules were obtained according to the brainstorming method. Using the non-replacement timing curtailed reliability test, it is calculated that the mean lifetime of the indicator does not meet the design lifetime of modules. The quality defect of the indicator which is the root cause of the flash/dark event of modules is determined. An experience feedback platform was established, and corrective actions and corrective measures were formulated to completely solve the problem that the event of the indicator flash/dark from the root cause.Through the effect tracking evaluation, the correctness of the application of the RCA method is verified.
