直肠癌Miles术后会阴切口的处理方式及护理对策

Treatment methods and nursing interventions of perineal incision after Miles operation of rectal cancer

目的比较直肠癌Miles术后会阴切口两种处理方式的利与弊,并为患者提供最佳护理对策。方法收集哈尔滨医科大学附属肿瘤医院从2015年8月至2016年10月收治的低位直肠癌患者184例的临床资料,统计分析其切口愈合情况及术后并发症。结果一期缝合组比开放填塞组愈合时间缩短,差异有统计学意义(t=18.97,P <0.05);感染发生率增加,差异有统计学意义(χ~2=5.705,P <0.05)。由于样本量不足,两组患者的疼痛程度、出血比例差异无统计学意义(P> 0.05)。结论直肠癌Miles手术会阴部切口的两种处理方式各有利弊,个体化治疗是最佳的选择。无论采取哪种处理方式,为了缩短愈合时间、减少术后并发症,均应采取相应的护理措施。

Objective Compare the advantages and disadvantages of two treatment methods of perineal incision after Miles operation, and provide the best nursing interventions for patients. Methods 184 patients were collected at the Third Affiliated Hospital of Harbin Medical University from August 2015 to October 2016. The wound healing and postoperative complications were statistically analyzed. Results The healing time of the primary suture group was shorter than that of the open packing group (t=18.97, P < 0.05), but the incidence of infection was higher (χ^2=5.705, P < 0.05). Due to the lack of samples, there were no significant differences in other aspects between the two groups (P > 0.05). Conclusions Two treatment methods of perineal incision after Miles operation have their own advantages and disadvantages. No matter which kind of treatment, in order to shorten the healing time and reduce postoperative complications, nursing interventions should be taken.
