128层螺旋CT低剂量技术在儿童副鼻窦检查中的应用 被引量:3

摘要 目的探讨128层螺旋CT低剂量技术(较低管电流)在儿童副鼻窦中的应用。方法选择2016年3月1日至2017年5月1日因临床怀疑为副鼻窦炎行CT检查的患者90例,随机分为三组,分别为甲组(常规剂量组)、乙组(低剂量组)、丙组(超低剂量组),每组30例,分别以240、20、10 m A的管电流对患者进行检查,管电流为240 m A为常规组,其余扫描条件均相同。结果甲组、乙组、丙组的图像评价为合格者分别是30/30、27/30、24/30,所得数据经多次配对设计两样本的卡方检验得出,乙组电流扫描所得图像与甲组比较,差异未见统计学意义(P> 0. 05),丙组与甲组比较,差异有统计学意义(P <0. 05)。乙组其余扫描参数不变的情况下所得图像,副鼻窦解剖结构的窦口及黏膜显示清晰,对疾病的诊断没有影响;丙组所得到图像噪声增加明显,6例副鼻窦解剖结构的窦口及黏膜显示模糊,影响了疾病的诊断。结论相较于常规扫描所用的管电流,低剂量(20 m A管电流) CT扫描所得图像可用于对鼻窦炎的诊断,低剂量CT对副鼻窦进行扫描可降低儿童辐射剂量。 Objective To investigate the application of child paranasal sinus treated with 128-layer lower tube current CT.Methods From March 1 st,2016 to May 1 st,2017,90 patients of clinicalsuspicion of the CT examination of paranasal sinusitis were selected,and they were randomly divided intothree groups because,group A(conventional dose group),group B(low dose group)and group C(ultra-low dose group),with 30 patients in each group.Each group were 240 mA,20 mA,10 mA tubecurrent examination of patients,the tube current of 240 mA for the conventional group,the rest of thescanning conditions were the same.Results Group A,group B,group C were evaluated as qualified,respectively,30/30,27/30,24/30.The data obtained after multiple paired design two samples of thechi-square test obtained group B tube current scan.There was no significant difference between the twogroups(P>0.05).Compared with the conventional tube of group A,the difference was not significant(P>0.05).There was significant difference in the tube current between group C and group A(P<0.05).Tube current of group B,the remaining scan parameters remain unchanged,the sinus andmucosa of the paranasal sinus anatomy showed no clear effect on the diagnosis of the disease.The noise ofthe image obtained when the tube current was group C increased significantly,and the number ofparanasal sinuses anatomical structure of the ostium and mucous membranes showed fuzzy,affecting thediagnosis of the disease.Conclusions Low-dose CT scans can also be used for the diagnosis of sinusitiscompared to the tube current used in conventional scans,so paranasal sinuses can be scanned clinicallywith low-dose CT(20 m A tube current),and can reduce radiationdoses in children.
