TDI-Tei指数评价慢性阻塞性肺疾病患者右心功能的研究

Evaluation of right heart function in patients with chronic obstruction pulmonary disease by TDI-Tei index

目的探讨组织多普勒(TDI)模式下所测右心心肌做功指数(Tei)评价慢性阻塞性肺疾病(COPD右心功能的价值。方法收集2017年6月至2018年3月明确诊断为COPD的患者113例作为观察组,并根据2017慢性阻塞性肺疾病全球倡议(GOLD)综合评估指标将其分为ABCD四组;另选取27例正常体检患者作为对照组。行常规超声心动图检查后于TDI模式下测取右心室Tei指数。结果 COPD患者右心舒张功能受损早于收缩功能; COPD患者右心室整体功能受损,并呈进行性发展;心率、年龄与右心室Tei指数无相关性。结论 Tei指数可作为右心室整体功能评估的高效指标,与COPD综合评估分组间相关性良好。

Objective To evaluate the right ventricular function in patients with chronicobstructive pulmonary disease(COPD)by tissue Doppler(TDI)myocardial work index(Tei).Methods From June 2017 to March 2018,113 patients with definite diagnosis of COPD were selectedas the observation group.According to the comprehensive evaluation index of 2017 Global Initiative forchronic obstructive pulmonary disease(GOLD),they were divided into ABCD four groups.Another 27 normal physical examination patients were selected as control group.After routine echocardiography,theright ventricular Tei index was measured in TDI mode.Results The right ventricular diastolic functionin COPD patients was earlier than that in systolic function.The overall function of the right ventricle wasimpaired in COPD patients and developed progressively.There was no relationship between the heart rateand age and Tei index of right ventricle.Conclusions The Tei index can be used as an effectiveindicator for evaluating the overall function of the right ventricle.The correlation is good with the COPDcomprehensive evaluation group.
