杨氏絮刺火罐联合电针治疗风寒痹阻型退行性腰椎管狭窄症的临床随机对照研究

A randomized controlled clinical study on treatment of degenerative lumbar spinal stenosis of wind-cold obstruction type with YANG’s pricking-cupping therapy combined with electro-acupuncture

目的观察杨氏絮刺火罐联合电针治疗风寒痹阻型退行性腰椎管狭窄症(DLSS)的临床疗效。方法将60例DLSS患者随机分为治疗组与对照组,每组30例。治疗组予杨氏絮刺火罐结合电针治疗,对照组予经皮神经电刺激疗法联合物理牵引。两组疗程均为8周,观察临床疗效,比较简化中文版腰椎管狭窄特异性评价量表(SC-SSS)评分的变化情况。结果①试验期间,治疗组脱落1例,最终完成试验者59例。②治疗组、对照组总有效率分别为89.66%、80.00%,组间总有效率比较,差异无统计学意义(P>0.05)。组间临床疗效比较,治疗组明显优于对照组,差异有统计学意义(P<0.05)。③组间治疗4周时及治疗8周时比较,症状严重度、生理功能评分差异有统计学意义,治疗组明显低于对照组(P<0.05)。结论与单纯物理疗法相比,杨氏絮刺火罐联合电针治疗风寒痹阻型DLSS的疗效更佳,可明显改善患者的腰腿痛及间歇性跛行,减轻下肢麻木。

Objective To observe the clinical efficacy of YANG’s pricking-cupping therapy combined with electro-acupuncture(EA)in treating degenerative lumbar spinal stenosis(DLSS)of the wind-cold obstruction type.Methods 60 patients with DLSS were randomized into treatment group and control group,30 patients in each group.Patients in the treatment group were treated with YANG’s pricking-cupping therapy combined with EA and those in the control group were given transcutaneous electrical nerve stimulation(TENS)combined with physiotherapy.8 weeks were regarded as a course of treatment.The clinical efficacy was observed.Changes in scores of simplified Chinese versions of Swiss Spinal Stenosis(SC-SSS)Questionnaire were compared.Results(1)During the trial,there was one drop-out in the treatment group,and 59 patients completed the trial.(2)The total effective rate was 89.66% and 80.00% in the treatment and control groups,respectively,with no significant difference between the two groups(P>0.05),Clinical efficacy in the treatment group was superior to that in the control group(P<0.05).(3)There were statistically significant differences in severity of symptoms and physical function score between 4 and 8 weeks of treatment,with significant decrease in the treatment group compared to those in the control group(P<0.05).Conclusion Compared with physiotherapy alone,YANG’s pricking-cupping therapy combined with electroacupuncture shows better efficacy in treating DLSS of the wind-cold obstruction type,significantly relieving pain of the waist and legs,improving intermittent claudication and alleviating numbness of the legs.
