心内科门诊军队伤病员焦虑抑郁状况横断面调查及影响因素

Cross-sectional survey of anxiety and depression of the army wounded and its influencing factors in cardiology outpatient department

目的:调查心内科门诊军队伤病员焦虑抑郁状态的发病现状及影响因素,为部队管理及心理干预提供依据。方法:采用横断面调查研究,选取2018-08-01～09-01就诊于我院心内科门诊的军队伤病员221名为研究对象,采用焦虑自测量表(self-rating anxiety scale,SAS)、抑郁自测量表(self-rating depression scale,SDS)判断军队伤病员焦虑抑郁状况。运用二分类Logistic多元回归分析影响抑郁、焦虑及二者共患状态的主要因素。结果:军队伤病员焦虑、抑郁评分均<60分,轻度焦虑、抑郁及二者共患发生率分别是33.9%、14.0%、10.4%。二分类Logistic多元回归分析示:睡眠障碍是焦虑(P=0.000,OR=35.892)、抑郁(P=0.002,OR=4.167)及二者共患状态(P=0.000,OR=46.992)的主要影响因素;面临退伍或工作调整是抑郁(P=0.002,OR=5.030)及二者共患状态(P=0.006,OR=6.242)的主要影响因素。结论:睡眠障碍是轻度焦虑、抑郁及二者共患状态的主要影响因素,军队伤病员面临退伍及工作调整也可引起焦虑抑郁,部队管理可针对不同人群及影响因素加强干预。

Objective:To investigate the status and influencing factors of anxiety and depression of military patients in the department of cardiology,and to provide the basis for army management and psychological intervention.Methods:Cross-sectional survey was used,a total of 221 army casualties continuously who visited the department of cardiology of Xinjiang general military hospital from August 1 st to September 1 st 2018 were selected.Self-rating anxiety scale(SAS)and self-rating depression scale(SDS)were used to determine the status of anxiety and deprression in military casualties.The main factors affecting depression,anxiety and comorbid state were analyzed by binary Logistic multiple regression model.Results:The anxiety and depression scores in military casualties were all less than 60,the incidence of anxiety,depression and comorbid state were 33.9%,14.0% and 10.4%,respectively.Binary Logistic multiple regression analysis showed sleep disorders was the main influencing factor of anxiety(P=0.000,OR=35.892),depression(P=0.002,OR=4.167)and comorbid state(P=0.000,OR=46.992).Retirement incidents or work change were the main influencing factors of depression(P=0.002,OR=5.030)and comorbid state(P=0.006,OR=6.242).Conclusion:Sleep disorders were the main influencing factors of mild anxiety,depression and comorbid state,retirement incidents or work change can cause anxiety and depression.Intervention in military management could be strengthened for different populations and influencing factors.
