The Efficacy and Safety of Plasma Laser or 2940nm Pixel Laser Combined with Medical Collagen Complex Gel Dressing in Treating Acne Scar

摘要 目的观察plasma等离子束与2940nm像素激光联合医用胶原复配型凝胶敷料治疗痤疮瘢痕的疗效与安全性。方法选取2017年9月-2018年9月在我科诊断为痤疮瘢痕患者120例,随机分为A、B、C、D四组各30例,A、B两组患者均采用plasma等离子束治疗,A组外用医用胶原复配型凝胶敷料,B组不予处理;C、D两组患者均采用2940nm像素激光治疗,C组外用医用胶原复配型凝胶敷料,D组不予处理。对每组患者的疗效及安全性分别进行对比。结果有效率:A组高于B组、C组高于D组,差异均无统计学意义;A组高于C组、B组高于D组,差异均有统计学意义;ECCA评分:A组治疗后ECCA评分低于B组、C组治疗后ECCA评分低于D组,差异均无统计学意义;A组治疗后ECCA评分低于C组、B组治疗后ECCA评分低于D组,差异均有统计学意义。术后反应及不良反应方面,AB、CD、AC、BD两组患者相比,视觉模拟评分法评分、疼痛持续时间、结痂脱落时间、急性炎症反应的半定量评分均有差异,且差异均有统计学意义;色素沉着发生率和皮肤敏感发生率均无差异。结论 plasma等离子束及2940nm像素激光对于痤疮瘢痕效果明显,且联合医用胶原复配型凝胶敷料后术后不适发生率降低,因此plasma等离子束或2940nm像素激光联合医用胶原复配型凝胶敷料的治疗方案值得推广。 Objective To observe the therapeutic effect and safety of plasma laser or 2940nm pixel laser combined with medical collagen compound gel dressing in the treatment of acne scars.Methods 120 patients with acne scars diagnosed in our department between September 2017 and September 2018 were randomly divided into four groups:A,B,C and D,30 cases in each group.A and B groups were treated with plasma beam,and C and D with pixel laser,with the A and C groups further treated with medical collagen compound gel dressing.The efficacy and safety of each group were compared.Results For therapeutic effects,Group A was better than Group B,Group C was better than Group D,the difference was not statistically significant;Group A was better than Group C and Group B was better than Group D,the difference was statistically significant.For ECCA score,Group A was lower than Group B,Group C was lower than Group D,both the differences were not statistically significant;Group A was lower than Group C,Group B was lower than Group D,the difference were statistical significant.In terms of postoperative reaction and adverse reactions,there were significant differences between A and B,C and D,A and C,and B and D groups in visual analogue score,pain duration,scab shedding time and semi-quantitative score of acute inflammatory reaction,while no significant differences were observed in term of the incidence of pigmentation and skin sensitivity.Conclusion The therapeutic effects of plasma and 2940nm pixel laser on acne scar are obvious,and the incidence of postoperative discomforts is reduced with the combination of medical collagen compound gel dressing.Therefore,the treatment plan of plasma or 2940nm pixel laser combined with medical collagen compound gel dressing is worth promoting.
