超声影像学在急性阑尾炎急查诊断中的应用价值

The value of ultrasound imaging in the emergency diagnosis of acute appendicitis

目的分析急性阑尾炎急查诊断中超声影像的应用价值。方法 2014年3月至2017年10月,于本院收治的急性阑尾炎患者中选取50例,所有患者均经手术与病理证实,临床中均行超声检查,对其诊断结果和影像学特征进行研究分析。结果经超声共诊断出单纯急性阑尾炎20例,急性化脓性阑尾炎10例,坏疽性和穿孔性阑尾炎8例,阑尾周围脓肿8例,诊断符合率为92%;单纯急性阑尾炎在超声下主要表现为阑尾肿大,短轴为"同心圆"状,长轴为"指状",急性化脓性阑尾炎表现为阑尾显著增粗,长轴为"腊肠"形,坏疽性和穿孔性阑尾炎的阑尾壁不具备清晰的轮廓,阑尾周围脓肿超声表现为有不规则混合型回声包块出现在回盲区。结论急性阑尾炎急查诊断中超声影像的应用价值显著,具有快捷方便、操作简单、准确率高等诸多优点,可将其广泛推广。

Objective To analyze the application value of ultrasound imaging in the acute diagnosis of acute appendicitis.Methods From March 2014 to October 2017,50 patients with acute appendicitis were enrolled in our hospital.All patients were confirmed by surgery and pathology.Ultrasonography was performed in the clinic.The diagnosis and imaging features were performed for research and analysis.Results Twenty patients with acute appendicitis,10 patients with acute suppurative appendicitis,8 patients with gangrenous and perforated appendicitis,and 8 patients with appendix abscess were diagnosed by ultrasound.The diagnostic coincidence rate was 92%;Simple acute appendicitis is characterized by enlargement of appendix under ultrasound,short axis is"concentric circle",long axis is"finger",acute suppurative appendicitis is characterized by significant thickening of appendix,long axis is"sausage"shape,and acute suppurative appendicitis is characterized by"sausage"shape.The appendiceal wall of gangrenous and perforated appendicitis does not have a clear outline,and the ultrasonic findings of periappendiceal abscess show irregular mixed echo masses in the ileocecal area.Conclusion The application of ultrasound imaging in the acute diagnosis of acute appendicitis is significant.It has many advantages such as quick and convenient,simple operation and high accuracy.
