维生素D与2型糖尿病肾病的相关性分析 被引量:6

摘要 目的探讨维生素D(Vit D)水平与2型糖尿病患者血糖、尿微量白蛋白、血清肌酐、肾小球滤过率(GFR)的关系。方法选取100例2型糖尿病住院患者,根据Vit D值,将其分为Vit D<20 ng·mL^(-1)组(n=41)及Vit D≥20 ng·mL^(-1)组(n=59)。对2组年龄、病程、血清肌酐、GFR、糖化血红蛋白(HbAlc),尿微量白蛋白进行比较。结果 2组病程、年龄、GFR值、HbAlc、血清肌酐比较差异均无统计学意义(P>0.05)。Vit D≥20 ng·mL^(-1)组尿微量白蛋白值低于Vit D<20 ng·mL^(-1)组(P=0.004)。相关性分析结果显示:Vit D水平与年龄、病程、血肌酐、GFR、HbAlc无关(P>0.05)。Vit D水平与尿微量白蛋白呈负相关(r=-0.316,P=0.001)。结论 Vit D缺乏可能与2型糖尿病肾病的发生及进展有关,且补充Vit D可能有助于2型糖尿病肾病的治疗。 Objective The investigate the relationships of vitamin D(Vit D) levels to blood glucose,urinary microalbumin,serum creatinine and glomerular filtration rate(GFR) in patients with type 2 diabetes. Methods A total of 100 inpatients with type 2 diabetes were divided into two groups:Vit D<20 ng·mL -1 group( n =41) and Vit D≥20 ng·mL -1 group( n =59).The age,disease course,serum creatinine,GFR,glycated hemoglobin and urinary microalbumin were compared between the two groups. Results There were no significant differences in age,disease course,serum creatinine,GFR and glycated hemoglobin between the two groups( P >0.05).The urinary microalbumin concentration in Vit D≥20 ng·mL -1 group was lower than that in Vit D<20 ng·mL -1 group( P =0.004).Correlation analysis showed that Vit D had no correlation with age,disease course,serum creatinine,GFR and glycated hemoglobin( P >0.05).However,Vit D was negatively correlated with urinary microalbumin( r =-0.316, P =0.001). Conclusion Vit D deficiency may be associated with the development and progression of type 2 diabetes nephropathy. Therefore,Vit D supplementation may contribute to the treatment of type 2 diabetic nephropathy.
