脊髓型颈椎病术后病人延续性护理需求量表的编制及信效度检验

Development and reliability and validity test of Continuous Nursing Demand Scale for Postoperative Patients with Cervical Spondylotic Myelopathy

[目的]编制适用于评估脊髓型颈椎病术后病人延续性护理需求的测量工具。[方法]通过查阅相关文献、半结构化访谈及专家咨询法构建条目池,选取81例脊髓型颈椎病术后病人进行预调查,采用收集到的数据进行项目分析和探索性因子分析,形成初始版量表。选取206例脊髓型颈椎病术后病人作为正式调查对象,评价量表的信度和效度。[结果]脊髓型颈椎病术后病人延续性护理需求量表包括15个条目,探索性因子分析提取3个公因子,累积方差贡献率为86.693%;量表的内容效度指数(S-CVI)为0.928。验证性因子分析模型拟合良好:χ~2/df=1.022,拟合优度指数(GFI)=0.946,比较拟合指数(CFI)=0.999,简效拟合优度指数(PGFI)=0.686,近似误差均方根(RMSEA)=0.010。量表的Cronbach′sα系数为0.905,Guttman Split-Half系数为0.961。[结论]编制的脊髓型颈椎病术后病人延续性护理需求量表具有良好的信度和效度,可用于脊髓型颈椎病术后病人延续性护理需求的评估。

Objective:To develop a measurement tool for assessing the continuous care needs of postoperative patients with cervical spondylotic myelopathy.Methods:By document retrieval,half-structurized interview and expert consulting,item pool was established.The survey was carried out on 81 postoperative patients with cervical spondylotic myelopathy.The collected data were used for item analysis and exploratory factor analysis to form the initial version.Totally 206 postoperative patients with cervical spondylotic myelopathy were selected for formal investigation objects to test the reliability and validity of the scale.Results:The Continuous Nursing Demand Scale for Postoperative Patients with Cervical Spondylotic Myelopathy consisted of 15 items,three common factors were extracted with exploratory factor analysis,and the cumulative variance contribution rate was 86.693%.The content validity index (S-CVI) of the scale was 0.928.The confirmatory factor analysis model fitted well χ^2/df =1.022,GFI=0.946,CFI=0.999,PGFI=0.686,RMSEA=0.010,the fitting degree of the model was good.The Cronbach's α coefficient of the scale was 0.905 and the Guttman Split-Half coefficient was 0.961.Conclusions:The Continuous Nursing Demand Scale for Postoperative Patients with Cervical Spondylotic Myelopathy has good reliability and validity.It can be used to evaluate continuous nursing demand of postoperative patients with cervical spondylotic myelopathy.
