中药熏蒸联合推拿治疗寒湿瘀阻证腰椎增生性脊柱炎的疗效观察

Efficacy of Chinese medicine fumigation combined with massage in treating lumbar hypertrophic spondylitis due to cold dampness and stasis syndrome

目的:探讨中药熏蒸联合推拿治疗腰椎增生性脊柱炎(寒湿瘀阻证)的临床效果。方法:选取2017年10月-2018年8月河南省洛阳正骨医院(河南省骨科医院)颈腰痛中心收治的120例符合纳入标准的腰椎增生性脊柱炎的患者为研究对象,按随机数字表分为研究组和对照组,每组60例,研究组给予中药熏蒸和常规推拿治疗措施,对照组给予安慰剂熏蒸和常规推拿治疗,疗程14天,分析两组患者在VAS、JOA等观察指标方面的变化情况并评定其临床疗效。结果:研究组临床控显率为95.0%;对照组临床控显率为83.3%,两组临床控显率比较差异具有统计学意义(P <0.05)。VAS评分:治疗后两组VAS评分具有下降,同组治疗前后比较差异有统计学意义(P <0.05)、(P <0.01),研究组VAS评分明显低于对照组,两组比较差异具有统计学意义(P<0.01)。JOA评定:两组治疗后JOA腰痛症状积分均降低。同组治疗前后比较差异具有统计学意义(P<0.05);且研究组JOA脊柱功能评分明显高于对照组,差异具有统计学意义(P <0.05)。结论:中药熏蒸联合推拿治疗临床效果优于推拿治疗措施,可有效改善患者的临床症状,提高治疗有效率,值得临床推广应用。

Objective:To explore the clinical effect of Chinese herbal medicine fumigation combined with massage in the treatment of lumbar spondylitis(syndrome of cold dampness and blood stasis).Methods:120 patients with lumbar spondylitis who met the inclusion criteria were selected from Luoyang Orthopedic Hospital of Henan Province(Henan Orthopedic Hospital)from October 2017 to August2018.They were divided into study group and control group according to the random number table,60 cases in each group.The study group was treated with Chinese medicine fumigation and conventional massage,while the control group was conducted by placebo fumigation and massage for 14 days.The changes of VAS and JOA in the two groups were analyzed to evaluate clinical effects.Results:The clinical efficiency of the study group was 95.0%and the control group was 83.3%.The difference between the two groups was statistically significant(P<0.05).After treatment,the VAS score of the two groups decreased,and there were significant differencesin the same group before or after treatment(P<0.05),(P<0.01).The VAS score of the study group was significantly lower than that of the control group,the difference between the two groups was statistically significant(P<0.01).The JOA scores of low back pain symptoms in the two groups decreased after treatment.There was a significant difference in the same group before or after treatment(P<0.05),and the JOA spinal function score of the study group was significantly higher than that of the control group(P<0.05).Conclusion:The clinical effect of Chinese medicine fumigation combined with massage is better than that of massage.It can effectively improve the clinical symptoms and improve the treatment efficiency.It is worthy of clinical application.
