彩色多普勒超声对小儿支气管肺炎临床治疗效果的评估研究

Evaluation of clinical effect of color Doppler ultrasound in children with bronchial pneumonia

目的探讨应用彩色多普勒超声动态观察肺部声像以评估小儿支气管肺炎临床治疗效果的可行性。方法以2017年9月～2018年9月接受治疗的患儿50例为研究对象,所有患儿均经胸部CT确诊为小儿支气管肺炎;患儿分别在治疗后1周完善肺部超声与胸部CT以评估临床治疗是否有效;以胸部CT的结果作为金标准,计算出彩色多普勒超声的灵敏度、特异度及约登指数,并根据CT结果决定是否改变治疗方案;治疗后2周再次复查肺部超声与胸部CT评估病灶是否完全消失,并计算出彩色多普勒超声的灵敏度、特异度及约登指数。结果治疗1周后复查肺部超声与胸部CT,发现彩色多普勒超声评估治疗效果的灵敏度为97.7%,特异度为80.0%,约登指数0.777;治疗2周后再次对比胸部CT与肺部超声,发现彩色多普勒超声评估病灶是否消失的灵敏度为100.0%,特异度为98.0%,约登指数0.980。结论彩色多普勒超声评估小儿支气管肺炎的临床治疗效果具有较高灵敏度和特异度,适合用于小儿支气管肺炎临床治疗效果的评估。

Objective To investigate the feasibility of using color Doppler ultrasonography to observe the changes of pulmonary sonography in the evaluation of clinical treatment of children with bronchial pneumonia.Methods 50 patients who underwent treatment in our hospital from September 2017 to September 2018 were enrolled in the study.All children were diagnosed as pediatric bronchial pneumonia by chest CT.The patients were improved lung ultrasound 1 week after treatment.Chest CT evaluation of clinical treatment is effective;using chest CT results as the gold standard,calculate the sensitivity,specificity and Yoden index of color Doppler ultrasound,and decide whether to change the treatment plan according to CT results;review again 2 weeks after treatment Pulmonary ultrasonography and chest CT were performed to assess whether the lesion completely disappeared,and the sensitivity,specificity and Yoden index of color Doppler ultrasound were calculated.Results After 1 week of treatment,ultrasound and chest CT were reviewed.The sensitivity of color Doppler ultrasound was 97.7%,specificity was 80.0%,and the index was 0.777.After 2 weeks of treatment,chest CT and ultrasound were compared.The sensitivity of Doppler ultrasound to assess whether the lesion disappeared was 100.0%,the specificity was 98.0%,and the approximate index was 0.980.Conclusion Color Doppler ultrasonography has a high sensitivity and specificity for the evaluation of clinical treatment of pediatric bronchopneumonia.It is suitable for the evaluation of clinical treatment of bronchopneumonia in children.
