基于赋权教育模式的自我管理计划对糖尿病血糖控制不佳患者的影响

Effect of Self-management Plan Based on the Empowerment Education Model on Diabetic Patients with Poor Glycemic Control

目的探讨基于赋权教育模式的自我管理计划对糖尿病血糖控制不佳患者自我管理行为和血糖控制水平的影响。方法 2017年3-12月,便利抽样法选取贵州省3所三级甲等综合性医院180例血糖控制不良的2型糖尿病患者为研究对象,按随机数字表法将其分为对照组和观察组。对照组按常规进行护理和随访,观察组实施基于赋权教育模式的自我管理计划,比较两组患者干预前后的自我管理行为及糖化血红蛋白水平。结果实施基于赋权教育模式的自我管理计划6个月后,观察组患者的自我管理行为得分高于对照组,糖化血红蛋白水平低于对照组,差异均有统计学意义(均P<0.05)。结论基于赋权教育模式的自我管理计划能够提高糖尿病血糖控制不佳患者的自我管理行为和血糖控制水平,是改善患者自我管理能力的有效方法。

Objective To explore the effect of a self-management plan based on the empowerment education model on self-management behavior and blood glucose control in diabetic patients with poor glycemic control.Methods From March to December 2017,180 patients with type 2 diabetes mellitus and poor glycemic control from three hospitals in Guizhou province were selected and divided into control group and intervention group according to the random number table method.The control group received routine care and follow-up,and the intervention group received a self-management plan based on the empowerment education model.The self-management behavior and the level of glycosylated hemoglobin(HbA1c)before and after the implementation were compared between the two groups.Results After 6 months application of the self-management plan based on the empowerment education model,the score of self-management behavior in the intervention group was higher than that of the control group,while the HbA1c was lower than that in the control group(all P<0.05).Conclusions Self-management plan based on the empowerment education model can improve the self-management ability and glycemic control level of diabetic patients with poor glycemic control,which is worthy of popularization and application.
