矢状窦旁脑膜瘤术中前1/2段及所属静脉损伤与信息处理速度相关性分析 被引量:1

摘要 手术治疗脑膜瘤是目前国内外首要推荐方式,经典理论认为,针对矢状窦旁脑膜瘤(PSM)患者术中位于上矢状窦(SSS)前1/3段旁脑膜瘤对SSS前1/3段予以结扎处理,对SSS中、后1/3段予以保护,对已证实SSS已完全闭塞中、后1/3 SSS旁脑膜瘤应行SSS结扎并切除。神经显微外科的不断发展,手术目的亦由术后保障生命向保护普通功能继而再向保存高级神经功能转变,目前越来越多学者着重研究过去认为"功能静区",切除对患者术后生活质量无影响与大脑皮层破坏可能引起癫痫发作、矢状窦及回流静脉处理等方面。本文就PSM患者术中SSS前1/2段和所属大脑上静脉(SCV)的损伤与信息处理速度(IPS)之间相关关系进行论述,对目前流行的SSS前1/3段结扎后对患者术后生活质量无明显影响的理念提出新观点。 Surgical treatment of meningiomas is currently the first recommended method at home and abroad.Classical theory holds that the anterior 1/3 segment of SSS should be ligated for parasagittal sinus meningiomas(PSM)in patients with superior sagittal sinus(SSS),and the middle and posterior 1/3 segment of SSS should be protected.SSS has been completely occluded.SSS ligation and resection should be performed for meningiomas adjacent to middle and posterior 1/3 SSS.With the continuous development of neuromicrosurgery,the purpose of surgery has also changed from ensuring life after operation to protecting ordinary functions and then to preserving advanced nervous functions.At present,more and more scholars focus on the study of"functional static area".Resection has no effect on the quality of life of patients after operation and the damage of cerebral cortex may cause seizures,sagittal sinus and venous reflux.The relationship between the injury of the first 1/2 segment of SSS and superior cerebral vein(SCV)and the information processing speed(IPS)in patients with PSM was discussed.A new viewpoint was put forward on the concept that the prevalent 1/3 segment of SSS after ligation had no significant effect on the quality of life of patients after operation.
