种植体周围炎中NF-κB的表达及其与RANKL的相关性初探

Expression of NF-κB in Peri-implantitis and Its Correlation with RANKL

目的:探究种植体周围炎中NF-κB的表达及其与RANKL的相关性。方法:选取本院2017年1月-2018年6月间行种植修复后发生种植体周围炎的患者45例(种植体61颗),纳入观察组;同时选择同时期行种植治疗且无种植体周围炎发生的患者45例(种植体52颗),纳入对照组。记录并对比种植体周围探诊深度(PPD)、牙槽骨丧失深度(DSB)、改良龈沟探诊出血指数(mSBI)和改良菌斑指数(mPLI),同时测定龈沟液中核转录因子(Nuclear factor-κB,NF-κB)和核因子受体活化因子配体(Receptor activator of NF-κB ligand,RANKL),并分析两者的相关性。结果:轻、中、重度种植体周围炎及健康种植体的PPD、DSB、mSBI及mPLI比较差异均有统计学意义(F=0.214,0.233,0.257,0.313;P=0.012,0.011,0.008,0.024),且各指标组内比较差异均有统计学意义(P<0.05);各组龈沟液中NF-κB、RANKL比较,差异均有统计学意义(F=0.872,1.017;P=0.001,0.000)。Pearson相关分析显示种植体周围炎龈沟液中NF-κB及RANKL存在正相关(r=0.631,P=0.000)。结论:种植体周围炎龈沟液中NF-κB表达显著增高,且与RANKL存在明显相关,其随炎症严重程度的增加而升高。

Objective:To explore the expression of NF-κB and its correlation with RANKL in periimplantitis.Method:From January 2017 to June 2018,45 patients(61 implants)with peri-implantitis after implant restoration were selected into the observation group,45 patients(52implants)with no peri-implantitis after implant restoration were selected into the control group.The periimplant detection depth(PPD),alveolar bone loss depth(DSB),modified sulcus bleeding index(mSBI)and modified plaque index(mPLI)were recorded and compared,the Nuclear Factor-KappaB(NF-κB)and the Receptor activator of NF-κB ligand(RANKL)in gingival crevicular fluid were measured and the correlation was analyzed.Result:There were significant differences in PPD,DSB,mSBI and mPLI in light,medium and severe periimplant inflammation and healthy implants(F=0.214,0.233,0.257,0.313;P=0.012,0.011,0.008,0.024),and there were significant differences within each indicator group(P<0.05).There were significant differences in NF-κB and RANKL in gingival crevicular fluid of each group(F=0.872,1.017;P=0.001,0.000).Correlation analysis showed that there was a positive correlation between NF-κB and RANKL in gingival crevicular fluid of peri-implantitis(r=0.631,P=0.000).Conclusion:The expression of NF-κB in gingival crevicular fluid is significantly increased,has a significant correlation with RANKL,and it related to the severity of inflammatory.
