联合应用不同氟喹诺酮药物治疗老年耐药肺结核的效果

Efficacy of the Combination of Different Quinolone Drugs in the Treatment of Older Patients with Drug-resistant Pulmonary Tuberculosis

目的探讨联合应用不同氟崖诺酮药物在老年耐药肺结核治疗中的临床效果:方法回顾性分析100例耐药肺结核患者的临床资料,随机均分为左氧氟沙星组和莫西沙星组,左氧氟沙星组采用左氧氟沙星和卷曲霉素为核心的化疗方案,莫西沙星组采用莫西沙星和阿米卡星为核心的化疗方案,记录并分析两组的临床有效率、病灶吸收率、痰阴转率及副反应发生率,结果治疗后2个月莫西沙星组的临床有效率、病灶吸收率及痰阴转率均显著高于左氧氟沙星组(P<0.05);治疗后24个月两组上述指标比较差异无统计学意义(P>0. 05);两组副反应发生率比较差异无统计学意义(P>0. 05)<结论莫西沙星和阿米卡星的联合化疗方案对耐药肺结核的早期疗效更为显著,安全性高。

Objective To evaluate the effect of the combination of different quinolone drugs on drug 一 resistant pulmonaiy tuberculosis in older patients. Methods The clinical data of 100 patients with drug - resistant pulmonan tuberculosis were retrospectively analyzed. The patients were randomly divided into levofloxacin group and moxifloxacin group, 50 cases in each group. In levofloxacin group, levofloxacin and capreomycin were used as the core chemotherapy regimen. In moxifloxacin group, moxifloxacin and amikacin were used as the core chemotherapy regimen. The efficacy, foci absorption, negative conversion of sputum and adverse events in two groups were recorded. Results The response rate, rate of foci absorption and rate of sputum negative conversion in moxifloxacin group were significantly higher than those in levoHoxacin group after 2 months of treatment ( P < 0. 05 ), but there were no significant differences between the two groups after 24 months of treatment ( P > 0. 05 ). There were no significant differences in the incidence of adverse events between the two groups (P>0. 05). Conclusion Combination regimen of nioxifloxacin and amikacin for drug resistant pulmonaiy tuberculosis is more effective and safe in the early stage of treatment.
