膝骨关节炎患者全膝关节置换术后影响膝关节功能恢复的因素分析

Factors Affecting the Recovery of Knee Function after Total Knee Arthroplasty in Patients with Knee Osteoarthritis

目的探讨膝骨关节炎患者全膝关节置换术后影响关节功能恢复的相关因素。方法选取2013年7月?2017年7月期间于我院接受全膝关节置换术的320例膝骨关节炎患者,根据术后恢复情况分为恢复理想组(165例)与恢复不理想组(155例),分别采用Spearman单因素及多元logistic回归分析术后影响关节功能恢复的因素。结果 Spearman单因素分析,术后膝关节功能恢复理想组与恢复不理想组在年龄、是否采用软组织平衡术、术后胫骨平台后倾角、术前HSS膝关节评分、术后并发症发生数以及术后视觉模拟量表(VAS)疼痛评分方面的差异均具有统计学意义(P<0.05);多元logistic回归分析,年龄、是否使用软组织平衡术、术前HSS膝关节评分、术后并发症及术后VAS评分均为术后关节功能恢复的相关因素.结论年龄、是否使用软组织平衡术、术前HSS膝关节评分、术后并发症及术后VAS评分是影响全膝关节置换术的膝骨关节炎患者术后膝关节功能恢复的高危因素。

Objective To investigate the factors affecting the recovery of joint function tifter total knee arthroplasty in patients with knee osteoarthritis. Methods 320 patients with knee osteoarthritis who undenvent total knee arthroplasty in our hospital from July 2013 to July 2017 were selected. The patients were divided inlo the ideal recovery group and the no ideal recovery1 group. Spearman single factor and multiple logistic regression were used to analyze the factors affecting the postoperative recovery of joint funclion. Results There were significant differences in age, soft tissue balancing, postoperative posterior tibial slope angle, preoperative hospital for special surgery ( HSS) knee score, incidence of postoperative complications and postoperative pain visual analog scale (VAS) between the two groups ( P < 0. 05 ). Multiple logistic regression analysis showed that age, soft tissue balanci ng, preoperative HSS score, postoperative complications and postoperative VAS score were related with postoperative joint function recovery. Conclusion Age, soft tissue balancing, preoperative HSS knee score, postoperative complications and postoperative VAS score are major factors which affect the postoperative function recovery of knee joint in patients with knee osteoarthritis after total knee arthroplasty.
