某基层综合医院住院患者医院感染调查分析

Investigation and analysis of hospital infections in hospitalized patients in a basic hospital

目的了解县级综合医院住院患者医院感染特点及相关因素,为加强医院感染管理提供依据。方法通过回顾性调查,对某县级综合医院2017年度住院患者医院感染病例进行统计分析。结果共调查26 504例住院患者,发生医院感染359例、375例次,医院感染发病率为1. 35%、例次发病率为1. 41%。医院感染率分居前4位的是ICU、神经外科、神经内科和呼吸内科,感染率分别为14. 69%、2. 84%、2. 51%和2. 28%。感染部位以下呼吸道最多见,占38. 13%。感染病原菌以革兰阴性杆菌为主,大肠埃希菌、肺炎克雷伯菌和鲍曼不动杆菌较为常见,革兰阳性球菌以金黄色葡萄球菌为主;多重耐药菌占40. 48%。结论该县级综合医院医院感染发病率处于较低水平,应重点加强高危人群、高危病区及高危因素的监控。

Objective To investigate the characteristics and related factors of hospital infections among inpatients in the county-level general hospital,so as to provide a basis for strengthening management of hospital infections.Methods The statistical analysis of 26 504 cases of inpatients with hospital infections in a county-level general hospital was done by means of the retrospective survey.Results Among 26 504 inpatients,there were 359 inpatients with 375 cases of hospital infections in total.The morbidity of hospital infections was 1.35% and the morbidity of cases was 1.41%.ICU,Neurosurgery,Neurology and Respiratory medicine were the top four of hospital infection rate with the rate of 14.69%,2.84%,2.51% and 2.28% respectively.Most of the cases were the lower respiratory tract infection with a rate of 38.13%.The gram-negative bacilli was the main pathogenic bacteria and Escherichia coli,Klebsiella pneumoniae,Acinetobacter baumannii,Staphylococcus aureus appeared commonly.Multiple resistant bacteria accounted for 40.48%.Conclusion Hospital infections rate in this county-level hospital is at a low level.Strengthening monitoring of high-risk groups,high-risk areas,high-risk factors should be focused on.
