摘要
目的选取冠心病患者、伴有高血压的冠心病患者和高血压患者各40例作为研究对象,研究血清可溶性尿激酶型纤溶酶原激活物受体与高血压心脏病患者左室射血分数、血浆脑钠肽和QRS宽度的关系。方法通过血清可溶性尿激酶型纤溶酶原激活物受体试剂盒对血清中的可溶性尿激酶型纤溶酶原激活物受体进行测定;采用十二导联心电图检测仪检查患者心电图,记录QRS宽度;通过化学发光微粒免疫分析仪对血浆脑钠肽进行测定;通过超声心动图仪进行超声心动图检查,记录左室射血分数。对患者的心脏结构功能、临床性质进行检测。结果 B组患者的血清可溶性尿激酶型纤溶酶原激活物受体(suPAR)水平最高,为(2.25±0.31);A组为(2.01±0.12);对照组为1.50±0.14(P <0.05)。对照组的左室射血分数(59.14±3.50)%较A组和B组大,而血浆脑钠肽(88.61±8.91)ng/L和QRS宽度(85.64±6.83)ms较A组和B组小;B组左室射血分数(41.30±3.81)%最小,血浆脑钠肽(378.45±16.58)ng/L和QRS宽度(125.16±3.42)ms最大(P <0.05)。FBG、ALT、AST、hs-CRP均为B组(4.54 mmol/L、28.27 U/L、22.30 U/L、4.18 mg/L)最小、A组(5.92 mmol/L、36.01 U/L、34.15 U/L、3.62 mg/L)最大,对照组处于两者间。B组患者的LVEDV(90.05±3.58)mL、LVESV(49.17±3.56)mL均较A组和对照组高,而E/A值(0.67±0.05)最低;A组数值大小处于B组和对照组间。B组患者的SBP(155.74±13.58)mmHg和DBP(100.17±10.96)mmHg均较A组和对照组高,6MWD(365.24±25.05)分相对较小;A组数值大小处于B组和对照组间。结论血清可溶性尿激酶型纤溶酶原激活物受体与高血压心脏病患者左室射血分数、血浆脑钠肽和QRS宽度间存在显著的负相关关系。
Objective In this study,40 patients with coronary heart disease,those with hypertension and 40 patients with hypertension were selected as research subjects to study the relationship between serum soluble urokinase plasminogen activator receptor and left ventricular ejection fraction,plasma brain natriuretic peptide and QRS width in patients with hypertensive heart disease. Methods The serum soluble urokinase -type plasminogen activator receptor was determined by removing the soluble urokinase-type plasminogen activator recepto kit. The patient′s electrocardiogram was examined by a 12-lead electrocardiogram detector,and the QRS width was recorded. The plasma brain natriuretic peptide was measured by chemiluminescence microparticle immunoanalyzer. The left ventricular ejection fraction was recorded by echocardiography. In addition,the cardiac structure function and clini- cal nature of the patients were tested. Results The highest level of serum soluble urokinase-type plasminogen acti- vator receptor(suPAR)was found in group B,group A,and control group(P < 0.05). The left ventricular ejec- tion fraction(59.14 ± 3.50)% of the control group was higher than that of group A and group B,while the plasma brain natriuretic peptide(88.61 ± 8.91)ng/L and QRS width(85.64 ± 6.83)ms were smaller than those of group A and group B. Left ventricular ejection fraction(41.30 ± 3.81)% was the smallest in group B,while plasma brain natriuretic peptide(378.45 ± 16.58)ng/L and QRS width(125.16 ± 3.42)ms were the largest(P < 0.05). FBG, ALT,AST and hs-CRP were the smallest in group B(4.54 mmol/L,28.27 U/L,22.30 U/L,4.18 mg/L),the largest in group A(5.92 mmol/L,36.01 U/L,34.15 U/L,3.62 mg/L)max,and the control group was between them. The LVEDV and LVESV of group B were higher than those of group A and control,while the E/A value was the lowest,and the value of group A was between group B and control. The SBP and DBP of group B were higher than those of group A(155.74 ± 13.58 mmHg)and control group(100.17 ± 10.96 mmHg),and the value of 6MWD (365.24 ± 25.05 points)was relatively small;the value of group A was between group B and control group. Conclusion There was significant negative correlation between serum soluble urokinase plasminogen activator receptor and left ventricular ejection fraction,plasma brain natriuretic peptide and QRS width in patients with hy- pertensive heart disease.
