后外侧小切口与传统切口人工全髋置换术的疗效比较

Comparison of curative effect between posterolateral small incision and traditional incision total hip arthroplasty

目的比较后外侧小切口与传统切口人工全髋置换术(THA)的手术效果。方法将我院收治的117例行后外侧小切口THA患者设为小切口组,同时将37例行传统切口THA患者设为传统组。比较两组的手术情况及术后恢复情况。结果小切口组的切口长度、手术时间、出血量及输血量均优于传统组(P<0.05)。两组患者I期愈合率均为100.0%。小切口组下地时间、术后2d C反应蛋白水平及血沉均优于传统组(P<0.05)。两组并发症总发生率无显著差异(P>0.05)。结论后外侧小切口THA入路简单精确,对软组织损伤较小,术后效果良好,安全性确切,但仍需严格把握适应证。

Objective To compare the operative effect of posterolateral small incision and traditional incision total hip arthroplasty(THA).Methods A total of 117 patients undergoing posterolateral small incision THA were set as small incision group and 37 patients undergoing traditional incision THA were set as traditional group.The operation and postoperative recovery situation were compared in both groups.Results The incision length,operation time,bleeding volume and blood transfusion volume of the small incision group were better than those of the traditional group(P<0.05).The healing rates ofⅠstage were 100.0% in both groups.The landing time,level of C-reactive protein and erythrocyte sedimentation rate at postoperative 2 d in the small incision group were better than those in the traditional group(P<0.05).There was no significant difference in the total incidences of complications between the two groups(P>0.05).Conclusion The approach of posterolateral small incision THA is simple and accurate,which has less soft tissue damage,good effect and safety,but it still needs to strictly grasp the indications.
