选择性超声造影引导下经皮肺穿刺活检对周围型肺病变的诊断价值

Dignosis value of percutaneous lung biopsy guided by selective contrast-enhanced ultrasound in peripheral lung lesions

目的探讨选择性超声造影引导下经皮肺穿刺活检在周围型肺病变诊断中的价值。方法将2016年6月至2017年6月在我院行常规超声引导下穿刺活检的疑似周围型肺病变的患者60例作为对照组,将2017年6月至2018年1月在我院行超声引导下选择性超声造影穿刺活检的疑似周围型肺病变的60例患者作为观察组。回顾性分析120例患者的临床资料,比较两组患者穿刺活检取材成功率及并发症发生情况。结果观察组的总取材成功率、符合选择性超声造影条件的患者的取材成功率均高于对照组(P<0.05)。结论针对不同患者及各自靶病灶特点选择性选用常规超声及超声造影的引导方式之组合能提高超声引导下经皮肺穿刺活检的成功率,并发症发生率无增加。

Objective To explore the value of percutaneous lung biopsy guided by selective contrast-enhanced ultrasound in the diagnosis of peripheral lung disease.Methods From June 2016 to June 2017,60 cases of suspected peripheral pulmonary disease accepted conventional ultrasound-guided needle biopsy in our hospital were selected as control group,from June 2017 to January 2018,60 cases of suspected peripheral pulmonary disease accepted selective contrast-enhanced ultrasound-guided needle biopsy were selected as observation group.The clinical data of 120 patients were retrospectively analyzed,and the success rates of needle biopsy and complications in the two groups were compared.Results The total successful rate of sampling and the successful rate of sampling in the patients who met the conditions of selective contrast-enhanced ultrasound in the observation group were higher than those in the control group(P<0.05).Conclusion Selective combination of conventional ultrasound and contrast-enhanced guidance acccording to the characteristics of different patients and their target lesions can improve the success rate of ultrasound-guided percutaneous lung biopsy without increasing the incidence of complications.
